Guide diagnosis, doctors and ladders; What do you mean?

Guidance is the person who consults in the outpatient hall of the hospital. A medical assistant is a person who helps doctors do odd jobs. I'm not sure about the guide ladder. It should mean the same as instructing doctors, and the work done is very poor.
