Announcement on receiving the teacher qualification certificate recognized by the Siyang County Education Bureau, Suqian City, Jiangsu Province in the first half of 2021

1. Recipients: Those who have passed the teacher qualification certification (see attachment).

2. Collection time: August 12-13, 2021 (9:00-12:00 am, 2:30-6:00 pm). Specific arrangements:

August 12:

Morning: Kindergarten teacher qualification certificate;

Afternoon: Primary school mathematics teacher qualification certificate;

August 13:

Morning: Primary school Chinese teacher qualification certificate, junior middle school teacher qualification certificate;

Afternoon: Other primary school subjects (excluding Chinese and mathematics), junior middle school teacher Qualification certificate.

3. Collection location: Room 407, Siyang County Education Bureau (No. 10, Beijing Middle Road, Siyang County).

4. Collection method: collect with the original ID card; fresh graduates collect with the original graduation certificate, copy and original ID card. If you entrust another person to collect it on your behalf, you must provide the original ID card of yourself and the entrustee, and a power of attorney.

V. Notes

1. The materials to be collected include: Teacher Qualification Certificate (1 copy) and "Teacher Qualification Certification Application Form" (1 copy). After receiving the "Teacher Qualification Certification Application Form", please deposit it in your personal personnel file in a timely manner and do not lose it.

2. During the collection period, please strictly follow the epidemic prevention and control requirements, wear a mask throughout the process, show your health code, accept body temperature monitoring, maintain a safe distance, and obey the management of on-site staff.
