Registration location

Candidates can register on-site at the appraisal station with their application materials before August 23. There are two appraisal stations in our province, namely Xiamen Health Talent Service Center and Fujian For Health Vocational and Technical College, candidates can choose their own identification station to register.

Application materials

Candidates need to submit the following materials:

1. Personal identification application form: signed by the candidate himself. The registration information cannot be changed after signature confirmation. Unsigned registration materials will be invalid.

2. Graduation certificate: original and copy. Candidates with a college degree or above must submit the electronic registration form for academic certificates from Xuexin.com.

3. ID card: original and copy.

4. Photos: 2 1-inch photos (format is jpg, size is between 15kb-45kb).

5. Training completion certificate: only submitted by candidates who are not majoring in medicine and health and candidates who have a technical secondary school degree in medicine and health.

Copies of all materials must be submitted in duplicate.

Registration method

1. Candidate individual registration: Submit paper application materials to the appraisal station, and package and send the electronic candidate information form (Attachment 2) and 1-inch photo to the appraisal station Station designated email. Electronic photos are named after "ID card number", and folders are named after "personal name + ID card number".

2. Training institution registration: Submit all candidate paper registration materials to the appraisal station, and package and send the electronic candidate information form and 1-inch photo to the designated mailbox of the appraisal station. Please fill in the subject of the email as "name of training institution + type of appraisal work". Each candidate's electronic photo is named after "ID card number", and the candidate information form must be filled in completely.
