What materials do I need to bring for the medical examination of studying in Australia?

1. Original and photocopy of the applicant's passport (1 photo page, 1 replacement page if the passport has been changed). If the passport has been submitted to the embassy, you can bring your ID card and a copy of it (different hospitals may have different requirements, so it is best to consult in advance);

2. Physical examination form;

3. Recent passport photos1-3;

4. If you have a history of chronic diseases, major diseases or operations, or are being treated now, please try to bring relevant medical records or valid medical certificates (such as medical records, hospital summaries, surgical records, pathological reports) and medication details during physical examination, and show them to the doctor.
