4. Fill in the form IM. 12 (signed by the applicant) -2 copies (original).
5. Color passport photos (2'x2)-4.
6. Copies of all page numbers of passports and travel documents-1 copy.
7. Overseas monthly income certificate -2 copies (original).
8. Copies of the asset certification documents listed in the application form-1 copy.
8. Copy of marriage certificate (if both husband and wife apply together) -2 copies.
9. Copy of birth certificate (if children apply together) -2 copies.
10. If the applicant's spouse or children are Malaysian citizens, please submit them together with their travel documents (including personal data and photo pages) and copies of their ID cards, as well as their marriage certificates (with their spouses) or birth certificates (with their children).
