Immigration with notarized household registration

The following materials shall be provided for birth notarization:

1, ID card, ID card, household registration book, passport and pass of myself and my parents;

2. If applying for agency, submit the agent's ID card and residence booklet;

3. Parents' marriage certificate. If the parents die, provide the death certificate issued by the public security department; If the parents divorce, they shall provide divorce certificates and divorce agreements or court judgments, conciliation statements and valid certificates;

4. Birth certificate issued by the village, residence or unit;

5. Personnel files of relevant units, original household registration certificates, birth certificates issued by hospitals, and one-child certificates;

6. If the notarial certificate needs to be affixed with photos, submit four two-inch bareheaded recent photos of myself, and attach a photo of the same background to the birth certificate.

The name on the child's birth certificate does not match the name on the household registration book. Parents of children are required to bring ID cards, household registration books, birth medical certificates and other materials to the police station where the children are registered to explain the situation, and issue certificates of the children's previous names or certificates of the same person.
