1. If a citizen applies for going abroad for personal reasons, he/she shall submit an application to the exit-entry administration department of the county or municipal public security organ where his/her permanent residence is located with his/her valid ID card, residence booklet or other household registration certificate and supporting materials corresponding to the reasons for applying for going abroad. Answer related queries and perform the following procedures:

(1) Submit a complete, clear and consistent Application Form for China Citizens' Ordinary Passport, and attach my recent bareheaded half-length colored white (or blue, gray) background photo, with the specification of 48MM×33MM, without taking photos with an imaging camera.

(2) Submit the original ID card, residence booklet or other household registration certificate and submit a copy;

(3) Submit supporting materials related to the reasons for applying for going abroad;

(4) Other supporting materials that the public security organ deems necessary.

Second, fill out the application form.

1. The Application Form for Ordinary Passport of China Citizens must be filled out item by item in block letters with pen or black ink. The project is complete, the content is true, and fraud is strictly prohibited.

2. The applicant's name, place of birth and date of birth must be consistent with the household registration book and identity card, and shall not be altered at will; The names of ethnic minorities should be filled in in the order of their identity cards.

3. Attach a photo that meets the requirements of going abroad (border), and it is required to be a recent photo of the applicant.

4 "domicile", "police station to which the domicile belongs" and "home address" must be consistent with the ID card and household registration book.

5, "work unit, work unit address, position, my identity", must be truthfully filled in, and shall not be simplified or omitted.

6 "going abroad" and "reasons for leaving the country" must be clearly filled in and shall not be altered at will.

7. "The number of times you applied for leaving the country for personal reasons" refers to the number of times you applied for leaving the country at the public security organ. If it is not approved, the reasons for not being approved shall be explained in the remarks column.

8 "application category" should be carefully filled in item by item according to different categories, and shall not be altered at will.

9 "main family members" should be filled in according to the actual situation. If the family members are overseas, the overseas address shall be filled in Chinese.

10, "applicant's signature" and "fingerprint collection" are conducted under the guidance of the accepting police.

1 1, the column of "comments of the unit or police station" shall be signed by the person in charge of the police station where the applicant works or has his/her household registration, and the comments shall be directly filled in the corresponding column of the application form with the official seal. Opinions include: whether the contents filled in the application form are true, whether the applicant is prohibited from leaving the country by law (see Part VI for details), whether the applicant is allowed to leave the country, etc. (If not, you must explain why). (3) College and middle school students are advised by their schools.

(4) Children under the age of 16 need to submit the certificate that the guardian agrees to go abroad, and the opinions of the unit or the police station are exempt.

Three. Proof materials related to the reasons for leaving the country.

According to the actual situation and work needs of entry-exit management in our region, and the different reasons for the applicant to go abroad, the corresponding certification materials must be provided:

1, visiting relatives and friends

To visit immediate family members: spouses, children, parents and parents of spouses, copies of residence certificates of the above-mentioned persons in the country of residence must be submitted.

When visiting non-lineal relatives or friends, an invitation letter from the foreign inviter and a copy of the residence certificate of the inviter in the country of residence shall be submitted.

Under the age of 16, they should be accompanied by their guardians, submit their opinions on agreeing to go abroad, submit their resident identity cards or household registration books for inspection, and submit a copy.

Step 2 settle abroad

If you are a dependent immigrant (resident), you must submit a residence permit issued by the competent department of the government of the country you intend to go to, and submit a copy. You must also submit a statement that the relatives who intend to go to the country agree to settle down and a copy of the valid identity documents of the relatives who intend to go to the country.

If a private service intermediary contacts to settle abroad (skilled immigrants or investment immigrants), it is required to submit a certificate of consent to settle issued by the competent department of the national government, as well as the business license and a copy of the business license of the private service intermediary, and submit a copy.

Where an adopted child goes abroad to settle down, it shall submit the adoption registration certificate issued by the civil affairs department at the first level of the autonomous region, the valid certificate, passport or other identity documents of the adopter, and submit a copy.

4. Studying abroad (including studying at one's own expense and studying at public expense)

If you contact to study abroad at your own expense, you must submit the standard admission notice issued by the receiving school and submit a copy. To study abroad through a legal intermediary, you must submit the standard admission notice of the receiving school and the Certificate of Self-funded Study Abroad Agency issued by the intermediary.

If it is a joint school run by two schools, if the application needs to complete a course outside the other school according to the agreement between the two schools, a copy of the agreement signed by the two schools or a business license issued with the approval of the Ministry of Education, as well as the original documents issued by the Academic Affairs Office and the Security Office of the applicant's school.

To study religious courses abroad (including in religious colleges), a certificate of consent issued by the religious department at or above the autonomous region level shall be submitted, and a copy shall be submitted.

Public students studying abroad (including those sent by the state and units) shall submit the approval documents issued by the Education Department of the autonomous region or the Education Bureau of the Corps and the certificate of consent to study abroad issued by their school, and submit a copy.

5. Going abroad for employment (including individual employment application and employment abroad introduced by overseas employment service agencies)

Individuals who contact abroad for employment or job hunting must submit the employment agreement of the country, the employer or the employer, or the legally binding employment (job hunting) contract signed by both parties, and a copy of the valid identity certificate of the legal person or employer of the proposed employer.

If it is contacted by a non-governmental employment agency, the business license and a copy of the business license of the non-governmental employment agency shall be submitted, and a legally binding contract or agreement signed between the non-governmental employment agency and the employees shall also be submitted. (At present, there is no employment agency in our district)

6. Labor service abroad

Submit the business license and business license (duplicate) of the labor service company with the right to operate overseas labor services, as well as the Approval Form for Overseas Labor Service Projects, the Registration Form for Overseas Labor Service Personnel and the List of Overseas Labor Service Personnel approved by the Department of Commerce of the Autonomous Region or the Bureau of Commerce of the Corps, and submit copies.

In addition, since June 5438+1 October1day, 2008, according to the notification requirements of the Ministry of Public Security, the Ministry of Commerce and the State Administration for Industry and Commerce, all expatriate labor companies must first go to the Exit-Entry Administration of the Public Security Department of the Autonomous Region for filing, and the Exit-Entry Administration of the Public Security Department of the Autonomous Region will notify the exit-entry administration department of the public security organ where the expatriate laborers are registered by fax, and then handle the documents for the applicants according to the Regulations on Exit-Entry Administration of People's Republic of China (PRC).

7. Going abroad on business

Enterprises, institutions or state-owned holding enterprises at or above the middle level who apply for going abroad to engage in trade negotiations and business activities shall submit the copies of the business license, tax registration certificate and organization code certificate of their units that have been examined annually, and submit the copies, as well as the dispatch letter of their units.

For self-employed, a copy of the annual business license and tax registration certificate shall be submitted. Self-employed persons only.

8. Others

(1) If it is a cultural exchange, the consent certificate issued by the cultural department at or above the autonomous region level shall be submitted.

(2) To engage in other unofficial activities abroad, you must submit the corresponding overseas invitation letter and a copy.

Four. Passport renewal

In any of the following circumstances, the holder may apply for a new passport:

1, the ordinary passport visa page is about to run out;

2. Ordinary passports are valid for less than six months, or for more than six months, but there are materials to prove that the validity period does not meet the requirements of the host country;

3. The name, gender, ID number, place of birth and date of birth on the household registration book have changed or the applicant's appearance has changed significantly;

4. The applicant can collect fingerprints after recovering from finger injury;

5. If a China citizen who has settled abroad returns to China for a short period of time to apply for renewal, he shall apply to the exit-entry administration department of the public security organ in his temporary residence, submit the original passport, proof of settlement abroad and the temporary residence permit issued by the public security organ in his temporary residence, and submit a copy.

6. Other circumstances identified by the Exit-Entry Administration of the Ministry of Public Security.

Sixth, passport replacement

To apply for a replacement of an ordinary passport, in addition to providing proof of the reasons for the replacement, it is also necessary to submit the corresponding supporting materials for the first application for an ordinary passport.

1. If a mainland resident loses his passport in China, the exit-entry administration department of the public security organ where his domicile is located shall be responsible for handling the passport replacement procedures. The applicant shall submit a report of loss and a description of the loss issued by the public security organ of the lost place.

2. If the passport is damaged and applied for reissue, the original damaged passport and the reason for the damage shall be submitted.

3. If a China citizen who has settled abroad loses his passport after returning to China for a short period of time, the exit-entry administration department of the public security organ in his temporary residence shall be responsible for handling passport replacement procedures. The applicant shall submit the certificate of loss reporting by the public security organ of the lost place and the certificate of overseas settlement.

Seven. Collection of fees and certificates
