Need to go to the designated hospital for physical examination. Generally, medical reports from non-designated hospitals are not accepted.

1. Original passport (valid passport) and two copies of passport (pages).

2 vaccination documents or written records about vaccination)

3 copies or abstracts of medical records, indicating the main medical problems and drug use at present.

4. Proof of disability or major injury.

5. Six clear 2-inch half-length photos with names on the back within six months.
