For other unavailable options, just tick "DoesNotApply".

Step 5: Fill in the address and telephone number (home address, mailing address, contact telephone number, email address, etc.). ).

Remarks 1: Whether the "mailing address" and "home address" of the filling person are consistent. If they are consistent, you can choose Yes, so you don't need to fill in the mailing address; If it is inconsistent, you should choose No, and then you should fill in the "mailing address" again. This is very important because this "mailing address" is the important information that the consulate will use when sending the visa and passport to the applicant after they are signed.

Note 2: When filling in the first part of the home address, please make sure that the information I fill in is consistent with the information in my passport. If this person's permanent address is not the address on the passport, you need to fill in the "mailing address".

Note 3: Fill in the "telephone contact information" and be sure to fill in the mobile phone number, because no one can guarantee to stay on the landline all the time. Without a valid mobile phone number, how can the visa office keep in touch with the person who fills it in?

Step 6: Personal passport information-passport number, issuing agency, etc.

It should be noted that the "validity period" of a passport should be at least 6 months. If you don't know the "passport number", you can ask the passport issuing agency and tell the person who filled it in. Besides, you should fill in "Have you ever lost or stolen your passport?" If yes, select "Yes" for backup, and fill in the number and issuing place of your previous passport. This is just a backup. Please fill in this form truthfully, because it will not affect whether your visa will be issued or not.
