5. Original and photocopy of ID card, graduation certificate, marriage certificate and other relevant documents;

6. If employees need to change their age, they must have a certificate issued by the unit agreeing to change their age, and attach a copy of the personnel department file of the unit with the official seal of the unit;

7. Birth certificate or birth medical certificate;

8. Household registration certificate;

9. Other supporting materials.

Article 17

When the contents of household registration need to be changed or corrected, the head of household or himself shall report to the household registration authority; If it is verified through examination, the household registration authority shall change or correct it.

When the household registration authority deems it necessary, it may require the applicant to issue a certificate of change or correction.
