Documents: baby's parents' ID card, marriage certificate, household registration book, birth medical certificate and supplementary page of birth medical certificate.

Process:

1. Parents hold ID cards of both parties and other relevant documents to the hospital where the new mother gave birth to get the birth medical records. This is to be filled out by the delivery room doctor, and it needs the supervision of the witness, and then the baby's parents need to sign it after confirming it is correct.

2. Parents hold ID cards, household registration books and birth medical records to the Maternal and Child Health Hospital where the new mother's household registration is located to obtain the birth medical certificate.

3. With both parents' ID cards, household registration books, marriage certificates, birth medical certificates and birth medical certificates, report and register the birth of the baby at the local police station where the new father or mother has permanent residence.
