Instructions for applying for a passport

(2) Fill in the application form:

1. When filling in the "Application Form", please note:

(2) The applicant’s name must be filled in using national standard simplified Chinese characters, consistent with the household registration book and resident ID card.

(3) The "pinyin name" must be spelled in Mandarin; the "date of birth" must be consistent with the household registration book and ID card; "birthplace" can be filled in with the province or municipality; "marital status" must be based on the actual civil affairs Fill in all parts.

(4) "Place of household registration" refers to the detailed address of the applicant's household registration location, which must be consistent with the household registration book.

(5) "Political outlook" can be filled in with "member of the Communist Party of China" or "member of the Communist Youth League" or "name of the democratic party" or "mass".

(6) "Education level" should be filled in according to the highest academic qualification recognized by the country's competent education department. "Occupation, position, and professional title" should be filled in according to the applicant's current and nationally recognized "professional title".

(7) The full name of "work unit" must be filled in. If the applicant's personnel file does not match the work unit, fill in the current work unit and indicate the file storage location in the remarks column. If the retiree's personnel files are stored in the original unit, fill in the original unit.

(8) "Number of times you applied to leave the country for private reasons" refers to the number of times you applied to leave the country at the public security organ. Those that are not approved will also be calculated together, and the non-approval situation will be explained in the remarks column.

(9) "Resume" should be filled in from junior high school, and the starting and ending dates should be accurate and consistent.

(10) "Main family members at home and abroad" should be filled in according to the actual Ministry of Civil Affairs. If family members are overseas, fill in the overseas unit and address in Chinese.

(11) Fill in the country of destination according to the first country after leaving the country (excluding transit countries). Only one reason can be selected.

2. Explanation on the opinions of the unit or police station:

(1) For national public servants, the organization and personnel department shall issue opinions according to the cadre management authority.

(2) Employees of state-owned and state-controlled enterprises and institutions shall have their opinions signed by the legal representative of the enterprise or institution or the personnel and security department of the unit authorized by them. If the legal representative applies to go abroad in person, the corresponding superior personnel shall Management issues opinions.

(3) For students over 14 years old (including 14 years old), the school will issue opinions. For those under the age of 14, their parents or guardians must provide proof of consent to go abroad, and they are not required to submit opinions from their work units or police stations. If the guardian is a grandparent, maternal grandparent or close relative, he or she must submit proof of kinship relationship, such as a kinship certificate issued by the police station or guardian unit, a household registration booklet that can prove kinship, etc.

(4) For persons other than the above categories (1) (2) (3), if the files are stored in the National Talent Exchange Center, Beijing Talent Exchange Center or Beijing Employment Agency Center, they can be issued by the "Center" Opinions; if the files are stored in the talent exchange center of the city or county, opinions may be issued by the "center" and the district (county) Personnel Bureau or Labor Bureau respectively. The talent exchange centers of various ministries and commissions and the Beijing Education System Talent Exchange Service Center approved by the national competent authorities can only issue opinions for internal mobile personnel of the system archived by the centers.

(5) If Beijing residents working in other places meet the provisions of Articles 1 and 2, their units shall issue opinions. If the files are stored with talents in other provinces, municipalities directly under the Central Government, or provincial capital cities (cities under separate state planning), the talents shall issue opinions.

(6) Other personnel shall issue opinions from the police station where Changxin’s registered permanent residence is located in Beijing.

(7) Temporary residents from other provinces (autonomous regions and municipalities directly under the Central Government) who have worked continuously for one year in foreign-funded enterprises, permanent representative offices of foreign enterprises and mixed-ownership enterprises in Beijing and have temporarily lived in Beijing for more than one year. If the company needs to go abroad for its business, the police station of the place of temporary residence, the applicant unit and the location of the files will issue opinions on going abroad. After verification by the Exit-Entry Administration Office and the public security agency of the place of residence, approval will be made in accordance with the law. The opinions of the unit or police station are valid for 6 months from the date of issuance.

3. Instructions on the corresponding supporting materials

(1) Personal household registration book, ID card, and household registration card for collective household registration. If the household registration book and ID card are basically inconsistent or the household registration is still being transferred, please wait until the police station where the household registration is located to go through the change or settlement procedures. If you move to Beijing and settle in other places, the place where the ID card is issued must be changed accordingly.

(2) Two recent straight-edged, frontal and colored single-person ID photos (one of which should be posted in the corresponding position on the application form).

(3) Depending on the reason for your going abroad, you also need to prepare the following certification materials: The certification materials are valid within 6 months from the date of issuance. If they are in a foreign language, they must be translated into Chinese by a designated translation company .

(3) Feedback application form

1. After the customer fills out the form and stamps it, return it to the travel agency, and then each business office will send professionals to compile it into the general office, and the general office will designate a dedicated person to input it into the computer.

2. After entering the information into the computer, each business office will designate a dedicated person to issue an invoice for the full travel expenses to the guest, and hand the application form and invoice to the guest together.

3. All business offices will promptly deliver the application form to the general office and discuss the submission of the form with the customer.

(4) Submit the application form: Guests should take the application materials (application form, invoice, barcode, personal ID card, household register) prepared by the hotel and go to the Exit-Entry Administration Bureau of the Public Security Bureau to apply for a passport. (The barcode is made in triplicate, with one copy on the cover sheet and the second copy submitted by the guest to the Public Security Bureau for passport use.)

(5) Passport collection: After the guest submits the application form, submit the "Private Exit Document" Return the application receipt to the travel agency and collect it from the General Office.

Southeast Asia:

1) Hold a five-year personal passport (please sign at the back of the passport) or a tourist passport valid for more than half a year.

Korea:

Guests who have held private passports for five years are not subject to the restrictions in the above areas. Guests holding private passports must provide their spouse’s name (nationality), employer, position, company phone number (proof of employment), home address and phone number.

Australia and New Zealand:

1) Hold a five-year personal passport valid for more than half a year (please sign at the back of the passport) and a one-time tourist passport.

2) Four 2-inch front-face photos without hats (please sign on the back) and two business cards.

3) Copies of documents: (A) Copy of the applicant’s ID card (B) Copy of each page of the household registration book of himself and his spouse/parents/children (if the applicant does not currently live in the place of household registration) , a copy of the temporary residence certificate is also required) (C) A copy of the marriage certificate (D) A copy of the retirement certificate.

8) If you are not traveling with your parents or only with one parent, you must provide a notarized certificate of kinship for the whole family and a notarized power of attorney for the parents who are not participating in the tour (the notarized certificate must be in Chinese and English) ). 9) Elderly people over 70 years old who want to go to the Gulf must provide two additional photos and submit them to the travel agency to collect the physical examination form. After getting the physical examination form, they must go to the designated place of the embassy Go to the hospital for a physical examination. The embassy will decide whether to issue a visa based on the results of the physical examination.

Note: Only Chinese citizens with permanent residence in Beijing, Shanghai, and Guangdong Province can use tourist passports to participate in Gulf tourism. Guests holding five-year private passports are not restricted in this area.

Book domestic return tickets

If the tour route you choose is to break up in Shenzhen or Guangzhou or the itinerary can make special arrangements if permitted, book the return ticket in advance. You can entrust a travel agency to purchase on your behalf or buy it yourself. Please ask the reception staff for the return date.

Exchange exit card

If you hold a private passport and the exit destination does not match your visa or the exit card is a yellow card, you need to go to the Exit-Entry Administration Bureau of the Beijing Municipal Public Security Bureau to apply.
