How to apply for a health check visa from Taiwan Province Immigration Department?

Eligibility for medical examination and tourist visa

1. Mainland compatriots who need general physical examination, medical beauty and outpatient treatment.

2. No province, occupation, age, criminal record or bad record in Taiwan.

Duration of visa use

Arrive in Taiwan Province within six months from the date of visa issuance; You can stay in Taiwan Province for up to fifteen days (depending on the contents of the visa).

Application fee: RMB 3,850.

List of materials required for visa application

1) Please fill in the basic information and declaration of entry permit passengers in detail.

2)2-inch bareheaded photo with white background. Color photos of the front half-length coated paper taken in the last 2 years, without colored glasses, with clear facial features and white background, cannot be modified or used as synthetic photos. It is recommended to send digital files of photos by email.

3) Copies of Chinese mainland resident ID cards (front and back) and other certificates or documents sufficient to prove identity.

4) Employment certificate (sample of employment certificate) or financial certificate (deposit certificate or annual income certificate or bank credit card gold card, choose one from three).

5) When the applicant is in the third region, a copy of the third region re-entry visa, the third region residence permit or the Hong Kong or Macao identity card should be attached.
