How to certify disabled people every year?

Generally, the village committee or community is responsible for the annual certification of the disabled. I need to bring my ID card, disability certificate and household registration book to the village committee or community for face-to-face certification. First, the conditions for handling the disability certificate

According to the disability assessment standard in the Disability Standard of the Second National Sampling Survey of Disabled Persons approved by the State Council, the applicant applying for the Disabled Persons' Card must meet one of the following qualifications and necessary conditions:

(1) visual disability: (eyes) must be those who cannot recover their vision through various drugs, surgery and other therapies (or those who cannot recover their visual function through the above therapies as determined by medical institutions).

(2) Hearing disability: (both ears) those who have not recovered for more than one year must be treated.

(3) Speech disability: refers to speech disorder caused by various reasons, which hinders normal speech communication, and the cause must be clarified. After treatment for more than one year, it still cannot be recovered.

(four) mental disability: must be a mentally ill person who has not recovered for more than one year.

(5) Intellectual disability: The intelligence is obviously lower than the average person's level, and it shows adaptive behavior disorder.

(6) Physical disability: refers to the human motor dysfunction caused by physical disability, deformity and paralysis. Disability assessment of limb injuries caused by diseases or traffic accidents, work-related injuries, accidents and other accidents must be restored after functional exercise for more than one year after final treatment (except amputation, paraplegia and joint fusion).

Second, the disability certificate handling process

1. The applicant submits an application to the Street Disabled Persons' Federation with his/her ID card, household registration book, four 2-inch recent bareheaded color photos and relevant hospital medical records.

2, the street full-time members according to the "People's Republic of China (PRC) disability assessment standards" of the applicant for preliminary examination, does not meet the disability assessment standards, inform the reasons for termination. To meet the disability assessment standards, truthfully fill in the Basic Information Acquisition Card for Disabled Persons, the Application Form for Disabled Persons' Cards in People's Republic of China (PRC) and the Disability Assessment Form in People's Republic of China (PRC), and put photos in the corresponding positions of the forms, and affix the official seal of the Disabled Persons' Federation. At the same time, the applicant will leave a contact number, and after the county disabled persons' Federation decides the specific date of disability assessment, the full-time Committee will promptly notify the applicant to participate in disability assessment.

3. After receiving the notice of disability assessment, the applicant holds the completed Basic Information Acquisition Card for Disabled Persons, People's Republic of China (PRC) Disabled Persons' Card Application Form, People's Republic of China (PRC) Disability Assessment Form and relevant medical records of the hospital, and queues up in the county disabled persons' federation according to the disability category, waiting for the corresponding doctor to make a medical appraisal of the disability level.

4. The disability assessment doctor assesses the applicant's disability level according to the disability assessment standard of People's Republic of China (PRC) (China). Those who meet the disability standards of the second national sampling survey of disabled persons shall make relevant records on the People's Republic of China (PRC) Disability Assessment Form, and those who do not meet the standards shall be returned.

5, the county disabled persons' Federation according to the doctor's disability assessment results for a preliminary summary, and the relevant information of the assessment form input into the basic information database of the disabled. If the disability assessment results meet the disability standards, fill in and print the relevant information of the disabled card according to the disability assessment results, affix the official seal of the filling and issuing organ in the column of filling and issuing organs, and report to the Municipal Disabled Persons' Federation for approval. Do not meet the disability standards or other requirements, returned, not for approval.

6. According to the disability standards and the Measures for the Administration of Disabled Persons' Cards, the Municipal Federation of Disabled Persons will review the preliminary opinions of the county-level Federation of Disabled Persons. Those who meet the disability standards shall be approved, and the official seal shall be affixed to the column of the approval authority, and the steel seal shall be affixed to the portrait of the holder. After the examination and approval of the Municipal Disabled Persons' Federation, the Disabled Persons' Card will be sent by the County Disabled Persons' Federation to the towns and street disabled persons' federations, and the full-time members of the towns and streets will inform the applicant to come and collect the Disabled Persons' Card.

Third, the disability certificate processing materials

1, a copy of the applicant's ID card;

3. Personal hospitalization medical records. If there is no original medical record and the disability status is unknown, the applicant shall first go to the hospital at or above the district level for examination, and provide inspection reports according to different disability categories:

Physical disability: outpatient medical records for more than one year, amputees, polio, and those with obvious retreat can be exempted from medical records;

Intellectual disability: IQ test report of Dean Hospital in Changzhou or 102 Hospital;

Mental disability: medical records of psychiatric hospitals at or above the district level (medical records treated for more than 2 years);

4. If the disability certificate is lost and reissued, a statement of loss shall be provided, which can only be reissued one month after its publication.

5. If you want to re-evaluate the disability after receiving the disability certificate, you must hold the certificate for one year before you can re-apply for re-evaluation.

4. What's the use of a disability certificate?

(1) For the disabled who have no source of livelihood, no ability to work, and can't set a supporter or a supporter, they will be fully subsidized according to the local urban and rural minimum living guarantee standard; For disabled families with a certain income, the difference subsidy shall be given according to the local urban and rural minimum living standard.

(two) to reduce the rural disabled voluntary labor, public welfare activities and other social burdens; For disabled people who have no ability to work, all kinds of administrative fees and collection fees are exempted; Persons with disabilities who have no ability to work, no source of income and no breadwinner shall be given priority in nursing homes (nursing homes and welfare homes).

(three) give priority to children with disabilities, children and adolescents with disabilities to attend kindergartens nearby; In the compulsory education stage, the school enrolls disabled children, teenagers and disabled children who enjoy the minimum living guarantee standard, which is proved by the local disabled people's Federation in towns and streets, and is free of tuition and fees; For the disabled and their children who go to school outside the district and county (city), if the economy is really difficult, the living expenses and transportation expenses shall be subsidized by the towns, streets and community neighborhood committees and villages with economic conditions, and the learning expenses shall be subsidized by the district and county (city); Disabled students who have obtained technical secondary school education (including technical secondary school) and have been rated as "three good students" by vocational high schools and technical schools (including school-level, district-level and municipal-level students) will be given a one-time subsidy according to the relevant provisions after being audited by the district and county (city) Disabled Persons' Federation, and the funds will be charged in the employment security fund at the corresponding level.

(4) Organs, organizations, institutions and enterprises above the township street level shall, according to the ratio of employees in their own units 1.5%, place disabled people with certain working ability for employment, give priority to employment when recruiting, and take care of their jobs and types of work, and implement equal pay for equal work. Enterprises and institutions shall, in accordance with the relevant provisions of the state and the province, try to avoid laid-off workers with disabilities in the process of restructuring. If the disabled workers are unemployed due to the bankruptcy of the enterprise, the enterprise should give priority to properly arranging the life of the unemployed disabled workers before bankruptcy.

(five) the disabled lease or contract arable land and mountainous areas for development and production, or lease or contract economic forests and agricultural products production and processing bases, and towns give preferential treatment in lease fees and contract fees. For disabled persons who apply for self-employment, the departments of industry and commerce, environmental protection and health shall give priority to the issuance of relevant business licenses within the prescribed time limit, and reduce or exempt 50% of the registration fees, training fees and license fees; Disabled individual industrial and commercial households with operational difficulties or losses are allowed to reduce or exempt individual industrial and commercial management fees, market management fees and other related expenses upon personal application and examination by the industrial and commercial departments. Villages, towns, streets and related units provide convenience for disabled people who are engaged in self-employed in venues, booths and booths. Persons with disabilities engaged in business activities may be exempted from all or part of the relevant taxes and fees upon personal application and approval of the tax authorities if they meet the provisions of national, provincial and municipal laws, regulations and policies on tax reduction and exemption. Persons with disabilities who rent public houses directly managed by the housing management department and engage in self-employment are exempt from 50% of the operating rent.

(six) disabled persons can enter public places such as museums, memorial halls, parks and temples below the municipal level free of charge with disability certificates; National Day for the Disabled (the third Sunday in May), International Day for the Deaf (the fourth Sunday in September), International Day for the Blind (10, 15) and International Day for the Disabled (12, 3) are free of charge, and the usual fee is halved; Sports facilities such as stadiums, gymnasiums and swimming pools (gymnasiums) are open to the disabled at preferential prices.

(7) Persons with disabilities who hold disability certificates seek medical treatment in towns (including towns) and above hospitals (health centers), and are exempt from registration fees and injection fees (excluding disposable material fees), giving priority to registration, payment, laboratory tests and medicine taking. Poor disabled people whose living standards are below the urban and rural minimum living security line will be given 30% bed fee, nursing fee and operation fee reduction and exemption in hospitals at or above the county level (excluding provincial level) for emergency treatment and hospitalization, which is proved by local disabled people's federations in towns and streets; Disabled people who do not enjoy the basic medical insurance for employees and have real difficulties in life will receive rehabilitation medical care such as cataract surgery and polio surgery. The expenses shall be subsidized as appropriate in the rehabilitation funds of the counties (cities) in the current year after my application, the certificate of the township and street disabled persons' federations, and the audit of the county (city) disabled persons' federations.

(8) Blind people can take urban and county (city) buses for free with the Disability Certificate (Hangzhou urban area can take the bus pass); Disabled people who take trains, ships and planes can give priority to buying bus (ship, plane) tickets with disability certificates, and bring necessary auxiliary devices free of charge. Public parking lots and roads are free of parking fees and road tolls for disabled vehicles with disability certificates, and road maintenance fees for disabled vehicles are free. The postal department delivers books and mail for the blind free of charge.

(9) Disabled people use public toilets free of charge with disability certificates.

(10) Installation of special cable TV lines and pipeline gas. For families with disabilities, if the application location for installation is consistent with their household registration location, the installation unit will reduce the installation fee and pipeline gas account opening fee by 50% on the basis of the certificate issued by Zhenjie Disabled Persons' Federation.

(1 1) If the children of urban disabled spouses are registered in rural areas, the public security department will give priority to their children under the age of 18 when "turning from rural areas to non-rural areas".

(12) Give priority to providing legal services and legal aid to the disabled. Provide free legal advice to the disabled and write legal documents for them. Disabled people who have enjoyed the minimum living guarantee are exempted from agency fees, notarization fees and other expenses with the proof of the District and County (city) Disabled Persons' Federation; Law enforcement departments should give priority to accepting applications and appeal cases of disabled people to ensure that their legitimate rights and interests are not infringed.

(13) During the period when disabled people participate in cultural, artistic, sports and vocational skills training, performances and competitions organized by people's governments at or above the county level or relevant departments, the wages and benefits of their units will remain unchanged; For the disabled who have no fixed income, the organizer gives subsidies.
