Announcement on Teacher Qualification Recognition in Autumn 20 19 in Xiaonan District, Xiaogan City, Hubei Province

1. Identity requirements: China citizens who have not reached the national statutory retirement age, whose household registration and residence (must have a local residence permit and be within the validity period) or their place of study (only for fresh graduates and graduate students, handled collectively by the school) are in Xiaonan District, Xiaogan City, Hubei Province.

2. Ideological and moral conditions: Abide by the Constitution and laws, love education, fulfill the obligations stipulated in the Teachers Law, and abide by teachers' professional ethics.

3. Education:

(1) To apply for kindergarten teacher qualification, one should have graduated from kindergarten normal school or above.

(two) to apply for the qualification of primary school teachers, you should have a college degree or above.

(three) to apply for the qualification of junior high school teachers, you should have a bachelor's degree or above in colleges and universities.

(4) Full-time normal education graduates who entered school on or before 2011year can be directly recognized (academic qualifications will not be upgraded), and the original and photocopy of qualification certificates for pedagogy, psychology and educational practice must be provided. Graduates majoring in part-time normal education before 20 1 1 must provide results that have passed the national teacher qualification examination and are within the validity period.

4. Professional requirements: All normal or non-normal graduates enrolled on or after 20 12 are required to take the national teacher qualification examination and obtain a written interview certificate. Among them, the subjects of kindergarten teacher qualification examination are comprehensive quality, knowledge and ability of teaching guarantee, and the subjects of primary school teacher qualification examination are comprehensive quality, knowledge and ability of education and teaching, and computerized examination is adopted. All written subjects of junior high school teacher qualification examination take paper-and-pencil examination. Only those who pass the written test can participate in the interview. (For details, please consult the Examination Center of Xiaogan Education Bureau. The results of the unified examination are valid nationwide.

5. Putonghua level: those who apply for the qualification of Chinese subject, general practice or kindergarten teacher must reach the second-class A and above of the national standard; Those who apply for the qualifications of teachers in other disciplines have reached the national standard of Grade II or above.

6. Physical condition: Have good physical and psychological quality, no history of infectious diseases and mental illness, and pass the physical examination in the county-level hospital designated by the teacher qualification accreditation body. Physical examination items must be completed in full, and the provincial people's hospital is the final inspection hospital in doubt.

Second, the appraisal procedure

1.

(1) Online registration: The registration time is from 201September 23rd to1October 23rd 10 at 7:00- 17:00 every working day (the online newspaper system will be closed during the National Day). Applicants should log on to "China Teacher Qualification Network" (). The information reported online must be true and accurate, and the consequences caused by false reporting or concealment shall be borne by the applicant.

(2) The applicant shall first check the information provided and submit the registration information after confirming that it is correct. After filling in the registration information, please print the Register of Applicants for Teacher Qualification in Hubei Province and Physical Examination Form (note: please do not change the format when downloading the form).

2. Site confirmation

On-site confirmation time is 2019101to 18 (effective working days). After the online registration is successful, those who apply for the qualifications of teachers in junior high schools, primary schools and kindergartens will go to Room 40 1 of Xiaonan District Education Bureau (No.238 Huaiyin Avenue, Xiaonan District, Xiaogan City) for on-site confirmation. Online registration can only take effect after on-site confirmation, and those who fail to make on-site confirmation are deemed to have given up automatically. Go to the designated hospital for physical examination after on-site confirmation.

The applicant shall submit the following materials when confirming at the site:

(1) Original second-generation ID card (within the validity period).

(2) Original academic certificate (if the academic information is compared with the electronic information in Xuexin.com, it may not be submitted). For academic qualifications that cannot be directly compared and verified (except secondary vocational school qualifications), the applicant must submit a report on China's higher education qualifications (online application in Xuexin.com), otherwise it will be deemed as unqualified academic qualifications and will not be accepted. Applicants are advised to verify their academic qualifications in Xue Xin. If it cannot be verified, please apply for a certification report in time.

(3) The original certificate of Putonghua proficiency test grade (which can be verified by the system, but not submitted).

(4) Personal books (download and print on the online registration interface, take photos with my signature, and upload according to the program requirements when filling in the registration information).

(5) Certificate of passing the examination (printed by the applicant on the website of the qualification examination for primary and secondary school teachers, but not submitted if the system can verify it).

(6) teacher qualification application form.

(7) If the applicant applies for recognition at the place where the household registration is located, submit the original household registration book or collective account; To apply for recognition in the place of residence, the original valid residence permit shall be submitted; If you apply for accreditation at the school location, submit the original student ID card (graduation certificate) with complete registration information.

(8) The original copies of the Physical Examination Form for Applicants of Primary and Secondary School Teachers' Qualification in Hubei Province and the Physical Examination Form for Applicants of Kindergarten Teachers' Qualification in Hubei Province are printed on A4 paper. (See Annex 1 and Annex 2)

(9) The applicant's recent 2-inch bareheaded color photo 1 photo (the photo of official certificate and the photo used to apply for teacher qualification certificate should be consistent with the photo submitted and uploaded online, and the name and ID number should be indicated on the back of the photo).

(10) List of teacher qualification certification materials in Xiaonan District of Xiaogan City (see Annex 3). Special reminder: For originals that cannot be automatically compared online, originals, photocopies and relevant certificates must be provided.

3. Physical examination.

After passing the on-site confirmation, applicants who meet the registration requirements for kindergarten teachers, primary school teachers and junior high school teachers will go to Xiaogan Chinese Medicine Hospital (Yong Dong Park East) for physical examination from 20 1919 to 8: 00 am on October 20 (those who cannot attend the physical examination on time due to personal reasons may be advanced or postponed appropriately). Take my ID card and physical examination form (with photos) during physical examination, and have an empty stomach.

4. The physical examination requirements for fresh graduates from universities and technical secondary schools are the same as above.

5. Other matters: Applicants are requested to make online declaration and on-site audit according to the specified time, place and requirements. If the application is not completed within the specified time due to reasons such as missing the application time, choosing the wrong accreditation body or on-site confirmation point, incorrect application information or incomplete submission materials, the accreditation body will no longer accept the application, and the responsibility will be borne by the applicant. Applicants who resort to deceit and cheat teachers' qualifications in the process of applying for accreditation will be punished by "revocation" or "loss" of teachers' qualifications according to relevant laws and regulations once verified. If the teacher's qualification is cancelled, the teacher's qualification shall not be restored within 5 years from the date of revocation; Those who lose their teacher qualifications may not re-apply for teacher qualifications.

6. Issue certificates. Eligible applicants for teacher qualification will receive the teacher qualification certificate from the accredited institution on February 30th, 20 19 to +2 1 30th, 2020 (effective working day).

7. Special statement: In view of the new information system launched by China Teacher Qualification Network, after this announcement is issued, our bureau will make appropriate adjustments and supplements to the relevant contents of the announcement according to the operation of the new system.
