First, determine the scope of the object.

Applicants for the qualifications of kindergarten, primary school and junior high school teachers in daxin county shall meet one of the following conditions:

(1) Having a household registration in daxin county;

(2) Holding a residence permit within the validity period of daxin county;

(3) Hong Kong, Macao and Taiwan residents who hold residence permits for Hong Kong, Macao and Taiwan residents within the validity period of daxin county may apply for recognition in their places of residence; Holders of the mainland travel permit for taiwan residents valid for 5 years and Hong Kong and Macao residents' passes to and from the mainland within the validity period, who take part in the qualification examination for primary and secondary school teachers in Guangxi Zhuang Autonomous Region and obtain a qualified certificate, shall apply for recognition at the place where the examination is held.

(4) Active servicemen and armed police of the troops stationed in daxin county.

Second, the authority of certification bodies.

Daxin county Education Bureau is responsible for the qualification of kindergarten, primary school and junior high school teachers in the county. The same applicant can only apply for teacher qualification once in the same year.

Third, the appraisal time.

Fourth, the identification conditions

(a) the applicant shall pass the national primary and secondary school teacher qualification examination, and pass the written examination and interview, and obtain the certificate of qualification examination for primary and secondary school teachers within the validity period.

If the applicant is a 202 1 publicly-funded normal student or a graduate student in education, he shall obtain a teacher's professional competence certificate for normal students, and it shall be within the validity period.

Applicants who enrolled in or before 20 1 1 year, and full-time normal college students who graduated in 20021year for reasons such as joining the army (the school kept their student status) may apply for direct recognition of their teaching qualifications in the same or similar disciplines.

(two) the applicant shall abide by the Constitution and laws, love education, fulfill the obligations stipulated in the Teachers Law, and abide by the professional ethics of teachers.

(three) the applicant should have the necessary knowledge to undertake education and teaching work and the ability to analyze and solve practical problems in education and teaching by using the knowledge learned.

(four) the applicant shall have the corresponding education as stipulated in the Teachers Law. Pre-school education (pre-school education) graduates from full-time secondary normal schools, secondary vocational schools and technical colleges may apply for the qualification of kindergarten teachers if they hold the certificate of qualification examination for primary and secondary school teachers within the validity period. This part of the staff must be filed in the Education Department of the autonomous region or the Department of Human Resources and Social Security, and those who have not filed can not be identified.

(five) the applicant should meet the standard of "Putonghua Proficiency Test Grade Standard" issued by the State Language Committee. Among them, those who apply for the qualification of non-Chinese teachers, whose household registration or work unit is in a town other than the resident of the people's government at the county level at the time of application, can have the third level of Putonghua.

(VI) According to the Standards and Measures for Physical Examination of Teachers' Qualification in Guangxi Zhuang Autonomous Region (Trial), they should have physical conditions that meet the requirements of engaging in education and teaching, pass the physical examination in the hospital designated by the teacher qualification accreditation institution and hold the Physical Examination Form for Teachers' Qualification in Guangxi Zhuang Autonomous Region (see Annex 3). Physical examination requirements are as follows:

1. All applicants in daxin county go to daxin county People's Hospital for physical examination;

2. If the above-mentioned applicant is not in the designated hospital for medical examination, it will not be accepted.

The applicant must check the following three items according to the actual situation when choosing the authorized institution and the on-site confirmation place: first, the place of residence; The second is the place of residence (you must apply for a local residence permit and fill in the residence within the validity period, or active servicemen and active armed police); The third is the location of the school (only for recent graduates, students studying in colleges and universities, graduate students).

(2) On-site confirmation.

After the online application is completed, the applicant shall go to the site for confirmation according to the time and place specified in the confirmation announcement published on the website of the corresponding authorized institution.

(2) The applicant recently took a color white background 1 inch photo, and clearly wrote his name and eight registration numbers assigned by the registration system on the back of the photo to apply for a teacher qualification certificate. The photos should be consistent with the photos uploaded during the online declaration.

(three) the corresponding materials of the applicant under different circumstances:

① daxin county household registration graduates need to provide the original household registration book (my page);

2 Graduates who hold residence permits within the validity period of daxin county must provide the original residence permit;

(4) The original "Physical Examination Form for Applicants for Teacher Qualification in Guangxi Zhuang Autonomous Region" (including relevant physical examination reports) issued by the hospital designated by the accreditation body (see Annex 3), and the conclusion on the physical examination form should be clearly filled in as "qualified" or "unqualified" (the validity period of the physical examination conclusion is 1 year), and stamped with the official seal of the physical examination hospital.

(5) The original certificate of Putonghua proficiency test level. If the information of Putonghua proficiency test is verified by the electronic information of "Teacher Qualification Management Information System", the original may not be submitted.

(6) Original academic certificate. Applicants who have obtained Hong Kong, Macao and Taiwan education should also submit the original certificate of Hong Kong, Macao and Taiwan education issued by the Study Abroad Service Center of the Ministry of Education, and those who have obtained foreign education should also submit the original certificate of foreign education issued by the Study Abroad Service Center of the Ministry of Education.

If the academic information is verified by the electronic information of "Teacher Qualification Management Information System", the original academic certificate may not be submitted. If the comparison is unsuccessful (except secondary vocational school education), the applicant shall submit the original academic certificate, as well as the China higher education certification report (which can be applied through Xuexin.com in advance), otherwise it will be deemed as unqualified academic qualifications and will not be accepted.

(7) To apply for the qualification of internship guidance teacher in secondary vocational schools, it is necessary to provide the original certificate of professional and technical position of assistant engineer or above or technical grade of intermediate workers or above.

(8) Those who have passed the system verification of the national primary and secondary school teachers' qualification examination certificate, 202 1 Education graduate students, and the Teachers' Professional Ability Certificate of public-funded normal students do not need to submit it on site.

(3) Audit by daxin county Education Bureau.

According to the evaluation opinions of the teacher qualification expert evaluation committee, the qualification conclusion is made within 30 working days from the date of termination of the application period, and the result is published on the portal website of daxin county People's Government of Guangxi Zhuang Autonomous Region.

(4) Issuing certificates.

After the accreditation body completes the on-site audit, it will make a conclusion according to the audit situation, and make a teacher qualification certificate for those who meet the accreditation conditions. The applicant shall, in accordance with the notification requirements of the accreditation body, obtain a teacher qualification certificate and an application form for teacher qualification accreditation. The application form for teacher qualification accreditation shall be submitted by the applicant to the management department where my personnel file is located and included in my personnel file, and the loss shall be borne by me. When fresh graduates receive teacher qualification certificates and teacher qualification application forms, they must also verify their academic qualifications. Anyone who has passed this certification must obtain the teacher qualification certificate from the daxin county Municipal Affairs Service Center, and the certification time will be notified to the applicant by the confirmation point.

(five) epidemic prevention and control requirements

According to the situation of epidemic prevention and control in COVID-19, daxin county Education Bureau strictly implements various prevention and control measures, reasonably arranges the wrong time and peak time, and makes overall plans for epidemic prevention and control and teacher qualification identification.

Other matters of intransitive verbs

(1) Strengthen study and publicity. County Education Bureau and accreditation bodies should seriously study, understand and master the relevant policies for the accreditation of teachers' qualifications, standardize the operation, recognize according to law, and protect the rights and interests of applicants. It is necessary to make overall arrangements for the evaluation work time within the jurisdiction during the opening period of the whole region, strengthen publicity through the Internet, newspapers and other media, timely release relevant information, patiently answer social inquiries, and ensure the smooth and orderly development of the qualification evaluation of primary and secondary school teachers. Institutions at all levels should strictly abide by the Network Security Law of the People's Republic of China in the use of information systems. Information system users must register with their real names, and user information must be reported to the Education Department of the autonomous region for registration, strictly implement the security responsibility system, and ensure information security.

(two) to strengthen the management of identification information system.

1. The offices of county education bureaus and institutions should pay more attention to the security management of information systems, and assign special personnel to be responsible for computers, USB flash drives and other items to prevent information such as user names, passwords, institutions, data and unpublished documents from being leaked. In the process of using information systems, we must strictly abide by the Network Security Law of the People's Republic of China, users of information systems must register with their real names, and user information must be reported to the Education Department of the autonomous region for registration, strictly implement the security responsibility system, and ensure information security.

2. The business unit room of the County Education Bureau should strengthen the information management of teacher qualification certification, and do a good job in the verification and cleaning up of certification data. For the data that do not meet the certification requirements but generate the certificate number incorrectly, the certificate number should be cancelled in the system in time before the end of this batch of certification. When publicizing the conclusion, the applicant's identity card information, contact information and other information items involving personal privacy shall not be disclosed. At the same time, in order to strengthen the management of teacher qualification certificate, it is necessary to refer to the application number of registered data in the information system and do a good job of testing the certificate before printing to prevent abnormal loss.

(3) Do a good job in inquiring and checking the teacher's qualification access.

1. According to the Notice of the Ministry of Public Security and other departments 12 on Printing and Distributing Opinions on Improving and Standardizing the Work of Issuing Certificates by Police Stations (Gong Tong Zi [20 16] No.21). The mainland applicant's proof of no criminal record shall be verified by the local teacher qualification accreditation body in the public security organ before making a conclusion, so as to avoid the situation that the applicant has committed criminal acts and revise the conclusion. The certificate that Hong Kong, Macao and Taiwan residents have no criminal record shall be issued by the applicant himself. When necessary, applicants from Hong Kong and Macao may apply for a letter from Chongzuo requesting the relevant departments to issue a certificate of no criminal record.

2. The business unit room of the county education bureau should communicate with the hospital undertaking the medical examination task in advance. The requirements are strictly in accordance with the Notice on Printing and Distributing the Standards and Measures for the Physical Examination of Teachers' Qualification in Guangxi Zhuang Autonomous Region (for Trial Implementation) (Gui Fa [2002]154), the Notice on Issues Related to the Cancellation of Hepatitis B Physical Examination for Teachers' Qualification (Gui Zhengfa [2010] No.34) and the Notice on Cancelling the Standards and Measures for the Physical Examination of Teachers' Qualification in Guangxi Zhuang Autonomous Region ()

3. Applicants can pay attention to relevant notices on the portal website of daxin county Municipal People's Government, and complete online application, on-site confirmation and physical examination within the specified time as required. If the application is not completed within the specified time due to reasons such as missing the application time, choosing the wrong accreditation institution or on-site confirmation point, incorrect application information or incomplete submission materials, the accreditation institution will no longer accept it.
