B. Application of dangerous vehicles

Applicant's information: name, telephone number, gender, ID number and visiting unit.

Access information: application type, visited institution (institution), visited name, visited telephone number, reason and visit time.

Vehicle information (driving license): model, license plate number, vehicle identification number, engine number and color.

Hazard information: scanned copy of business license of transport enterprise, scanned copy of ID card of legal representative of transport enterprise, scanned copy of driver's license of dangerous goods transport vehicle and scanned copy of qualification of escort.

Materials required for filing hazardous chemicals transport vehicles (bring the following materials to the No.1 Squadron of Daya Bay Petrochemical Comprehensive Management Brigade for filing)

1. Vehicle information registration form must be filled in and printed as required (handwriting is invalid, and the form is provided by Daya Bay Petrochemical Comprehensive Management Brigade).

2. Both sides of the identity of the legal representative of the company shall be stamped with the company seal.

3. A copy of the business license stamped with the company seal.

4. A copy of the business license (dangerous goods) stamped with the company seal.

5. Copy of driver's license, ID card, work permit and ID card, and affix the company seal.

6. A copy of the escort ID card of the escort, with the official seal of the company stamped on the front and back.

7. Copy of vehicle driving license and transportation certificate. Copy of the front and rear (trailer) road transport certificate and affix the official seal of the company.

8. Vehicle compulsory insurance. Copy of commercial insurance policy stamped with the official seal of the company.

9. A copy of the carrier's liability insurance shall be stamped with the official seal of the company.

Three. Matters needing attention in application and review

1. The institution visited is a public institution, and the wrong filling will lead to the failure of the audit. If it is wrong, please contact the registration center to correct it.

2. The applied vehicle has a legal license plate number (the license plate number cannot contain special symbols such as spaces, dots and horizontal lines, and the application for agricultural vehicles is not supported).

3. Audit time of registration center (8: 30- 12: 00, 14: 30- 18: 00).

Four. Reasons for failure to submit application

A. Prompt information: The return visit time for construction and maintenance shall not exceed one month. Please confirm whether the filled time for seeing a doctor is within 30 days.

B. Prompt information: You cannot apply repeatedly within the validity period. Please confirm whether the license plate is applied repeatedly during the application period.

6. How to check the application progress?

Enter the application page, click the application progress query, and the query window will appear: enter the license plate number and accept.

Log in with the account of enterprise administrator, click the enterprise audit in the park in the navigation menu, click Approved in the lower left corner, enter the license plate number to query, click Query, select the single number to view, and click View.
