According to the spirit of the Notice of Hubei Provincial Department of Education on Doing a Good Job in the Implementation of the Qualification System for Primary and Secondary School Teachers in our Province in 20021year, combined with the actual situation of our county, the relevant matters concerning the qualification of teachers in our county in the spring of 20021year are hereby announced as follows:

First, the appraisal time

1. Online time:

The first online registration time in spring is May 6, 7: 00-May 18 17:00 (mainly for applicants who meet the registration requirements and have obtained the certificate of teacher qualification examination and are within the validity period, full-time college graduates, etc. Among them, graduate students in education and normal students at public expense who hold the Certificate of Teachers' Professional Ability have started to register since May 12).

2. On-site confirmation time:

The first on-site confirmation time in spring: May 7-May 19 (effective working day).

Second, the scope and object of identification

1. China citizens, active servicemen and armed police stationed in Changyang Tujia Autonomous County, who have not reached the national statutory retirement age, and whose household registration and place of residence (must have a local residence permit and are within the validity period), such as education graduate students and public-funded normal students who have obtained primary and secondary school teachers' qualification certificates and are within the validity period or meet the recognized conditions, or full-time normal graduates who have enrolled in or before 201year.

2. According to the Notice of the Secretary Management Department of the Hong Kong and Macao Affairs Office of the State Council of the Secretariat Bureau of the Taiwan Province Work Office of the Central Committee of the Ministry of Education on Relevant Issues Concerning Hong Kong, Macao and Taiwan Residents' Application for the Qualification of Primary and Secondary School Teachers in the Mainland (19), Hong Kong, Macao and Taiwan residents who study, work and live in our county have no criminal record and can live and hold residence permits for Hong Kong, Macao and Taiwan residents. The academic qualifications, other conditions and procedural requirements for applying for teacher qualification are the same as those for mainland (mainland) applicants.

Third, the identification conditions

1. Education: The qualified education for applying for kindergarten teacher qualification is that the kindergarten teacher has graduated from a normal school or a junior college or above, and the pre-school education major of a secondary normal school or a secondary vocational school approved by the provincial education administrative department can be regarded as qualified education (E.J.Z. [2015] No.6). When applying for teacher qualification, the graduates majoring in normal education who entered 20 1 1 are not required to upgrade their academic qualifications for the time being. That is, the qualifications of primary school teachers are secondary normal school graduates and above, and the qualifications of junior high school teachers are junior college graduates and above. When applying for teacher qualification, graduates majoring in normal education who entered school on or after 20 12 should be upgraded accordingly. That is to say, the qualification conditions for applying for primary school teachers are junior college or above, and the qualification conditions for applying for junior high school teachers are junior college or above (E.J.Zi. 2. Putonghua requirements: Applicants for Chinese, primary school and kindergarten teacher qualifications must meet the second-class A and above standards, and applicants for other disciplines must meet the second-class B and above standards.

3. Physical conditions: The physical examination standards for applicants are strictly in accordance with the "Physical Examination Standards and Measures for Applicants for Teacher Qualification in Hubei Province" (E Teacher [2002] No.3). The physical examination of the personnel applying for kindergarten teacher qualification shall be carried out in accordance with the provisions of the Notice of the Ministry of Education on Adjusting the Physical Examination Standards of the personnel applying for kindergarten teacher qualification. Physical examination items must be completed, and the physical examination conclusion is "qualified".

Four. recognition program

(1) Online declaration.

1. Eligible applicants should log on to China Teachers' Qualification Network (www.jszg.edu.cn) within the specified time and declare from the "Report Portal of Teachers' Qualification Network". 2. Precautions:

(1) Correct selection of authorized institutions and on-site confirmation points. The accreditation bodies and on-site confirmation points for applying for kindergarten, primary school and junior high school teachers' qualifications all choose "Changyang Tujia Autonomous County Education Bureau", and the selection of "Yichang Education Bureau" and "Changyang Tujia Autonomous County Education Bureau" for applying for internship guidance teachers' qualifications in senior middle schools, secondary vocational schools and secondary vocational schools make on-site confirmation points.

(2) Sign and upload personal books correctly. Applicants download personal books from China Teacher Qualification Network and print them on A4 paper. Personal books will be integrated into the teacher qualification application form. Please sign and date in block letters at the "person", and then scan or take photos to upload. (2) On-site confirmation.

1. On-site confirmation location: If the applicant's household registration or residence (with residence permit and within the validity period) is in Changyang Tujia Autonomous County, go to the Teacher Management Unit of Changyang Education Bureau for on-site confirmation (Room 4-408, Huanglong Road, Longzhouping Town 16), and contact the telephone number 0717-533508/Kloc-0.

2. On-site confirmation must carry the following materials:

(1) The information list of applicants for teacher qualification certificate is in duplicate. (See Annex 1 for the sample table. Write down the name and date of the calligraphy, and print the rest. )

(2) Original second-generation ID card or household registration book (original residence permit). Servicemen and armed police should provide military officer's cards or police officer's cards when applying at the place of service. If it is proved that the service place cannot be displayed, the personnel relationship certificate issued by the subordinate unit or the personnel department of the unit organization shall be provided.

(3) Two recent photos of official documents with white background 1 inch bareheaded color (used to apply for teacher qualification certificate, which should be the same as the photos uploaded when applying online, with the name and ID number indicated at the bottom of the back of the photos);

(4) the original academic certificate. Hong Kong, Macao and Taiwan academic degrees should also be submitted to the original certificate of Hong Kong, Macao and Taiwan academic degrees issued by the study abroad service center of the Ministry of Education, and the original certificate of foreign academic degrees issued by the study abroad service center of the Ministry of Education should also be submitted. In the process of material review, for the academic qualifications that cannot be directly compared and verified by the national information system (except the academic qualifications of secondary vocational schools), the applicant needs to submit the academic qualifications report of China Higher Education (applying for www.chsi.com.cn online in Xuexin.com), otherwise it will be regarded as unqualified academic qualifications and will not be accepted. Applicants are advised to verify their academic qualifications in Xue Xin. If it cannot be verified, please apply for a certification report in time. To apply for the qualification category of internship instructor in secondary vocational schools, it is also necessary to provide the title certificate equivalent to assistant engineer and above or the technical level qualification certificate of intermediate and above workers.

(5) The original certificate of Putonghua proficiency test level.

(6) Certificate of Teachers' Qualification Examination or Certificate of Teachers' Professional Ability of Normal Students.

If the above certificate fails to pass the verification of "Teacher Qualification Management Information System", a copy shall be submitted after on-site verification.

Applicants who have graduated from full-time normal education and enrolled at or above 20 1 1 year, who did not take part in the national unified examination, should provide the original and photocopy of the qualification certificate of pedagogy, psychology and educational practice, as well as the photocopy of the applicant's information page on the college admission roster. A copy must be stamped with the official seal of the archives management unit, and marked with the original, the agent and the contact telephone number.

After the on-site confirmation is completed, all the original certificates will be returned to the applicant, and the materials required for confirmation will be put into a kraft paper file bag by the applicant, and the catalogue of the data bag will be collected and pasted on the spot and handed over to the certification body for retention and delivery.

(3) physical examination.

The period from the start date of on-site confirmation of each batch to the end of on-site confirmation time of each batch is the validity period of this teacher qualification physical examination. Applicants who meet the basic conditions for accreditation at the site shall receive a physical examination form at the confirmation site and go to the designated hospital (Changyang County People's Hospital) for a physical examination for teacher qualification accreditation. All physical examination items must be completed. The conclusion of physical examination is "qualified", and the physical examination form signed by the doctor and stamped with the official seal of the physical examination hospital is the effective physical examination result. On the day of physical examination, you are not allowed to eat any food (including drinks) before the blood test is completed. After the physical examination is completed, send the physical examination form to the on-site confirmation point within the specified time. Not using the prescribed physical examination form, not in the designated hospital, and not submitting the physical examination form within the specified time is considered invalid.

(4) authentication.

Those who meet the corresponding teacher qualification conditions shall be recognized, and the corresponding teacher qualification certificate and teacher qualification recognition application form (hereinafter referred to as "one certificate and one form") shall be issued. Verb (abbreviation of verb) matters needing attention

1. Applicants can only apply for accreditation and obtain one teacher qualification certificate each year. The applicant shall make online declaration and on-site confirmation in accordance with the prescribed time, place and requirements. If the applicant fails to complete the application within the specified time due to reasons such as missing the application time, choosing the wrong accreditation institution or on-site confirmation point, incorrect application information or incomplete submission materials, the accreditation institution will no longer accept the application, and the responsibility will be borne by the applicant.

2. Individuals will be recorded in the national credit information platform, and application materials such as "personal book" must be filled in carefully and truthfully. The applicant's proof of no criminal record shall be verified by the public security organ through data comparison or letters. If the public security organ verifies that the applicant has a criminal record, it will not be recognized and issued. Applicants who resort to deceit and cheat teachers' qualifications in the process of applying for accreditation will be punished by "revocation" or "loss" of teachers' qualifications according to relevant laws and regulations once verified. If the teacher's qualification is cancelled, the teacher's qualification shall not be restored within 5 years from the date of revocation; Those who lose their teacher qualifications may not re-apply for teacher qualifications.

3. The Putonghua proficiency test grade certificate shall be subject to the national Putonghua training and testing information resource network/certificate inquiry, and those that cannot be detected shall not be accepted.

4. Those who apply for the qualification of internship guidance teachers in senior middle schools (secondary vocational schools) and secondary vocational schools should log in to Yichang Education Bureau (/) and operate according to 202 1 Announcement on Confirmation of Teachers' Qualification in Spring Senior Middle Schools in Yichang. Applicants with household registration and residence in Changyang Tujia Autonomous County will be confirmed by Changyang Education Bureau on the spot. The time, place and requirements are the same as those of junior high schools, primary schools and kindergarten teachers. If they fail to do this, they will be regarded as having given up.
