III. Materials to be submitted when applying for registration

(1) One copy of the Application Review Form for Nurse Practice Registration.

(2) One copy of the applicant's ID card (printed on the same page A4).

(3) One original and one copy of the applicant's academic certificate.

(4) One original and one copy of the certificate of passing the nurse qualification examination.

(5) One original and one copy of the health examination certificate issued by the medical institution designated by the competent health administrative department.

All submitted materials shall be printed and copied on A4 paper (the front and back of the ID card are printed on one page), bound into volumes in the listed order, packed in a file bag according to the requirement of one bag for each person, and the name of the unit and the name of the applicant shall be indicated on the cover of the file bag.
