Three. Registration procedure and time

In 20 14, the way of online registration and on-site confirmation was still adopted for the junior and intermediate professional pharmacy qualification examinations in the province. Candidates go to the designated confirmation point in the city where the work unit is located to go through the on-site confirmation procedures (the setting of the confirmation point is determined by the Municipal Food and Drug Administration as appropriate). Candidates from central units stationed in Anhui and provincial units stationed in Hefei go through the on-site confirmation procedures at the US Food and Drug Administration.

(1) Online Registration

Time: May 12 to May 22nd.

Please follow the following procedure:

1. Log on to Anhui Food and Drug Talent Network (www.ahfda.com), and click "Online Registration" for the junior and intermediate technical qualification examination of pharmacy in the province to enter the main page of candidate registration.

2. Fill in personal information. Fill in the candidate's basic information on the "Fill in Personal Information" page, in which the items marked with "*" must be filled in, and the written content cannot exceed the limited number of characters. Candidates who have taken the 20 13-year intermediate professional and technical qualification examination for pharmacy and passed some subjects can roll to 20 14 and continue to apply for the remaining subjects of the 20 14-year intermediate qualification examination.

3. Fill in the exam subjects. Select the category and account you want to apply for in the Select Category and Account for Application page.

4. Upload electronic photos. The photo requirements are: a recent two-inch (34×45 mm) bareheaded photo in jpg format, with the size below 100Kb.

The provincial US Food and Drug Administration will review the electronic photos submitted by candidates. Candidates can check whether the submitted photos are qualified on the third day after online registration. The photo is unqualified, please resubmit it as required.

5. Print the exam registration form. Those who pass the photo review can print the registration form and commitment letter of the 20 14 Anhui Pharmaceutical Primary Technical Qualification Examination, which must be printed on both sides.

6. If you encounter any difficulties in operating the online registration procedure, please bring your electronic photo and register with the help of the local US Food and Drug Administration. Candidates are responsible for the accuracy of information.

(2) On-site confirmation

Candidates will go to the designated place of the local food and drug administration for on-site confirmation with relevant materials from May 24 to May 29, and accept the joint qualification examination of the local food and drug administration and the Human Resources and Social Security Bureau, and will not be accepted after the deadline (the specific time and place of on-site examination in each city will be determined by the municipal food and drug administration). Pay the examination fee on the spot after passing the examination. Anyone who is found to be unqualified in the exam will be disqualified; Those who practise fraud will be dealt with severely according to relevant regulations. After on-site confirmation, candidates are not allowed to change their majors and subjects.

The materials to be submitted for on-site confirmation are:

1. 1 copy of "Anhui province 20 14 year junior and intermediate technical qualification examination registration form" (print the commitment letter on the back and sign it);

2. My valid ID card and photocopy (the original will be returned after verification on the spot, and the photocopy will be signed by the auditor).

3. Original and photocopy of graduation certificate (the original shall be returned after verification on the spot, and the photocopy shall be signed by the auditor). Fresh graduates are required to bring the academic certificate issued by the school and submit the original academic certificate to the local food and drug administration for review before August 3 1. Those who fail to provide the original academic certificate within the time limit will be disqualified from the examination.

4. The original and photocopy of my pharmacy professional technical qualification certificate and employment certificate (excluding pharmacists and Chinese medicine practitioners; The original shall be returned after verification on the spot, and the copy shall be signed by the auditor).

5. I have two recent two-inch bareheaded photos with the same background (which must be consistent with the uploaded electronic photos).
