Large-scale achievements in handling the collapse accident of Tuojiang Bridge in Fenghuang County, Hunan Province;

On August 3, 2007, at about 6: 45/kloc-0, a bridge collapsed under the Tuojiang levee in Fenghuang County, Hunan Province, causing 64 deaths, 4 injuries and 18 minor injuries, resulting in a direct economic loss of 39,747,000 yuan.

(1) Basic

The Totti River Bridge project is a construction project of Fengda Highway from Fenghuang County, Hunan Province to Daxing Airport in Tongren, Guizhou Province, and it is an important control project. The total length of the bridge is 328.45 meters, the deck width is 13M, the longitudinal slope is 3%, the bridge span is a four-hole 65m articulated arch bridge, and the other sections are catenary. The pier is 33 meters high and is a stone arch bridge. In June 2003, Hunan Provincial Department of Land and Resources approved the preliminary design of Fenghuang large-scale expressway project, and in February of the same year approved the report of Fengda Expressway project. Tuojiangdi Bridge started on March 2, 2004/kloc-0, and the planned construction period is 16 months. At the time of the accident, the filling materials for the soffit, side wall and arch of the bridge have been basically completed, and the demolition work is nearing completion. It is planned to complete all the bridge construction projects by the end of August 2007 and open to traffic on September 20 as a gift for the 50th anniversary of Xiangxi Autonomous Prefecture.

Construction unit: Xiangxi Phoenix Highway Construction Co., Ltd. (hereinafter referred to as "Phoenix Company"), a state-owned enterprise of Xiangxi Autonomous Prefecture People's Government. Design and geological exploration unit of public Huagang Research Institute affiliated to Changsha University of Science and Technology. The Institute has the "Engineering Design Certificate" in the highway industry, with the grades of "Engineering Consulting Qualification Certificate" and "Engineering Survey Certificate".

The construction unit is Hunan Road and Bridge Construction Group Company (hereinafter referred to as "Bridge Company"). It is a large state-owned enterprise, a 28-minute company and a joint-stock enterprise (unit). Awarded by the Ministry of Construction as "General Contract of Super Highway Engineering Construction, Specialized Contract of Highway Subgrade Engineering, Specialized Contract of Highway Pavement Engineering, Specialized Contract of Bridge Engineering, Specialized Contract of Highway Traffic Safety Facilities Engineering" and "Qualification Certificate of Construction Enterprise", and obtained the "Safety Production License" in July 2006. "Under the road and bridge management system, the two institutions are responsible for the specific construction tasks of the seven road companies in Tuojiang Jody.

Supervision unit: Hunan Jinqu Traffic Consulting and Supervision Co., Ltd. is owned by 45 natural persons. Qualification grade of highway engineering supervision.

(2) The cause of the accident

1。 The direct cause of the accident

As the main arch of the bridge, the masonry material does not meet the specification and design requirements, the superstructure of the bridge is unreasonable in the construction process, and the masonry quality of the main arch ring is poor, which reduces the integrity and strength of the masonry structure. With the increase of the load on the arch, the whole bridge collapsed rapidly under a broken arch close to the number 0, that is, the strength of the second abdomen limited the influence of the collapsed masonry arch effect.

2。 The accident mainly

First of all, the project manager of Tuojiang Bridge in Dagongdi Road No.7 Company and the construction unit Feng Highway and Bridge Construction Corporation changed the construction scheme of the main arch without authorization, and the site management was chaotic, and the construction of the main arch ring did not meet the specifications. When the main arch ring did not reach the design strength, the construction work of withdrawing the frame began. Second, the construction unit Xiangxi Phoenix Highway Construction Co., Ltd. (referred to as "Fengda Company"), the project management is chaotic, and the construction unit is not seriously urged to rectify the construction quality problems found. Without consent, the design unit and the construction unit blindly reversed the construction period without changing the original design and construction scheme of the main arch, and even needed to supervise the bridge inspection. Iii. Engineering Supervision Hunan Jinqu Traffic Consulting and Supervision Co., Ltd. failed to prevent its owner's arch construction unit from changing the construction plan without authorization, and found the problem of quality supervision and rectification of the poor construction quality of the main arch ring, but did not measure the information of the completed main masonry arch and signed it for acceptance. Fourth, Huagang Research Institute, a design and geological prospecting unit, subcontracted exploration projects to individuals in violation of regulations, and the depth of geological survey and design was not enough, and the on-site service and design disclosure were not in place. Fifth, the quality supervision departments of traffic bridge projects in Hunan and Xiangxi have seriously neglected their duties in quality control.
