Many contents of this accident case can be found in our daily work. Let's review several events that happened in our factory:

(1) 1987 17 At 8: 00 on July, the #2 machine was shut down to improve the low-cycle self-start construction of the high-frequency cutting machine. During the process of restoring the hot standby at 18: 30, when the #2 machine's water turbine was put into operation and the electric operation protected the power supply, the power supply melted, which seriously violated the safety regulations of worker Lin XX. In the process of finding out the cause of the fuse, the positive sequence low-voltage overcurrent tripping 182, 183 switches, # 1 main transformer 1 10kv side and system of #2 unit were disconnected, resulting in a non-evaluation accident.

(2) Ernst & Young line protection annual inspection on September 24th, 183, 184 switch, ct, pt, etc. Cooperate with power failure to carry out preventive test. Wang xx, the relay worker, was paralyzed in safety thinking, seriously violated the safety work regulations, and did not strictly implement the working ticket system and the work guardianship system. When restoring the connection between pt and ct secondary terminal circuits, he did not follow the safety regulations.
