What are the requirements for applying for the certificate of safety supervision bureau?

Conditions for applying for the special operation certificate (obtaining evidence) of the Safety Supervision Bureau:

(a) at least 65438 08 years of age, and not more than the national statutory retirement age;

(2) Having passed the physical examination in a community or medical institution at or above the county level,

(3) Having a junior high school education or above;

(four) have the necessary knowledge and skills of safety technology;

(five) other conditions stipulated in the corresponding special operations.

Application materials for special operation certificate (evidence collection) of the Safety Supervision Bureau:

(1) copy of the front of the ID card 1 copy, and the second-round exam students need to provide a copy of the old card.

(2) Two recent 1 inch color photos.

(3) a copy of junior high school education or above or a copy of the household registration book.

(4) Fill in the Application Form for Special Operation Examination (Individual), as shown in the attached table 1.

(5) Fill in the Letter of Commitment on Integrity of Special Operation Assessment, as shown in Schedule 2.

(6) Fill in Schedule 3 of Personal Health Commitment.

(7) Trainees under review shall fill in Schedule 4 Special Operation Instructions (Individual).

Special operation certificate issued by the Safety Supervision Bureau:

Students who take the special operation certificate (obtaining evidence) examination and pass the examination will get the People's Republic of China (PRC) Special Operation Certificate supervised by state administration of work safety within 35 working days. The certificate is used nationwide, and the first trial lasts for three years.

Review and renewal of special operation certificate:

After submitting materials and originals according to the new evidence collection, they will pass the examination and replacement after participating in relevant training and assessment.

Special operation certificate examination instructions:

(1) There are two exams for new students: the theoretical exam and the practical exam. Review students have an exam: theoretical exam.

(2) Passing scores: theoretical test (80 points) and practical test (80 points)

(3) One person can only arrange one exam on the same day.
