Application of venous indwelling needle in pediatric nursing

Objective To explore the application method and effect of intravenous indwelling needle in pediatric nursing. Methods 120 pediatric patients admitted to our hospital from February 4, 1965 to February 5, 1965 were selected as clinical research objects, and the patients were randomly divided into observation group and control group, with 60 cases in each group. Children in the control group were given routine care, while children in the observation group were given intravenous indwelling needle care on the basis of routine care. The nursing effects of the two groups were compared. Results The success rate of puncture and nursing satisfaction in the observation group were significantly higher than those in the control group, and the difference was statistically significant (P < 0.05). Conclusion The application of venous indwelling needle in pediatric nursing can significantly improve the success rate of puncture and the nursing satisfaction of children, which is worth popularizing.

Pediatrics are all young children, and their physiological development is not perfect, so the blood vessels are slender; In addition, many children have poor self-control during infusion due to the influence of diseases. The above two characteristics lead to the situation that nurses often cannot puncture pediatric patients successfully at one time, but need to puncture repeatedly [1]. Repeated puncture will damage the blood vessels of children and lead to disputes between nurses and patients. The method of infusion puncture needs to be improved. In recent years, intravenous indwelling needle has been used more and more in pediatric clinical infusion, and has shown many advantages. In order to clarify the role and influence of intravenous indwelling needle in pediatric nursing, our hospital specially selected 120 children as clinical research objects for special research, and now the results are reported as follows.

1 data and methods

1. 1 general information. This study * * * selected 120 pediatric children, and the admission time in our hospital was from February 20 14 to February 20 15. Patients were randomly divided into observation group and control group, with 60 cases in each group. Observation group: 33 boys and 27 girls; The age ranged from 65438 0 to 65438 065438 0 years old, with an average age of (3.77 65438 0.34) years old. The hospitalization time was 65438 0 ~ 8 weeks, with an average of (4.2 65438 0.8) weeks. Control group: 3/kloc-0 boys and 29 girls; The age ranged from 65438 0 to 65438 02 years, with an average age of (3.69 65438 0.55) years. The hospitalization time was 65438 0 ~ 9 weeks, with an average of (4.5 65438 0.5) weeks. After comparison, there was no significant difference in general data between the two groups (P & gt0.05).

1.2 method. Children in the control group were given routine care. The children in the observation group were given intravenous indwelling needle care on the basis of routine nursing: first, choose appropriate blood vessels and indwelling needles. When choosing to puncture blood vessels, nurses should try to choose thicker, straighter and more elastic blood vessels as the puncture site of indwelling needle, and avoid blood vessels with joints and no venous valves. In addition, because of the high incidence of phlebitis in lower limbs, it is best not to choose veins in lower limbs as puncture sites. When choosing intravenous indwelling needle, commonly used indwelling needle with model 18~24G is generally selected [2]. Second, the fixation and puncture of venous indwelling needle. First of all, before puncture, the child takes a supine position with his head sideways; After that, the nursing staff selected the appropriate head blood vessels and disinfected the puncture site with Maokang iodine. Then, the nursing staff tightened the heparin cap of the indwelling needle, grasped the needle seat and soft seat tube of the indwelling needle, and removed the needle tip protective sleeve of the indwelling needle; Keep the angle between the needle holder and the vein at15 ~ 30, insert the needle directly, observe the inflow of blood, send the hose into the vein, then take out the needle core and put it into the sharp instrument storage box; Turn on the infusion switch, check whether the infusion is smooth, then cover the puncture with sterile film, and then wrap it with breathable paper tape twice for fixation. Third, seal the pipe. Generally speaking, normal saline is generally used for pediatric positive pressure sealing. The method is as follows: take 3 ~ 5ml of tube sealing liquid and slowly inject it into the lumen through the infusion needle, first inject 2 ~ 3ml, then pull out the infusion needle while injecting, but a few needles must be left in heparin cap. The speed of injection and needle withdrawal should be slow to ensure that the tube sealing liquid fills the lumen. When only 0. 1~0.2 ml of sealing liquid remains, clamp the extension tube with a small clamp, and then completely pull out the needle [3]. Fourth, keep time. Under normal circumstances, it should be kept for 3~5 days, but in order to avoid complications such as phlebitis and infection, it is recommended to keep it for 3~4 days.

Clinical observation and evaluation of 1.3. Observe the puncture situation of the two groups of children and record the success rate of puncture; The self-made questionnaire was used to investigate the nursing satisfaction of the two groups of children. If the children could not sue by themselves, their families would answer instead.

Two results

2. 1 puncture comparison results. All 60 children in the observation group were successfully punctured at one time, and the puncture success rate was 100%. In the control group, 49 cases were successfully punctured once, and 4 cases were successfully punctured again, with a puncture success rate of 88.33%. By comparison, the success rate of puncture in the observation group was significantly higher than that in the control group, and there was a significant difference (χ 2 = 5.45438+04, P < 0.05).

2.2 nursing satisfaction comparison results. The nursing satisfaction of the children in the observation group was 95.00%, which was significantly higher than that of the children in the control group, and there was a significant difference (P < 0.05).

3 discussion

The reason of nurse-patient disputes in pediatrics is often that infusion puncture does not meet the requirements of children and their families, especially repeated puncture. Pediatric children have thin blood vessels and poor fit. After routine puncture, puncture will always be unsuccessful, which will damage children's small blood vessels. In order to solve this problem, another infusion method-intravenous indwelling needle was used in clinic. This study discussed the application of intravenous indwelling needle in pediatric nursing, and compared the nursing effect of intravenous indwelling needle nursing with that of routine nursing. The results showed that the success rate of puncture and nursing satisfaction of children with intravenous indwelling needle were significantly higher than those of the control group. The nursing of venous indwelling needle in this study includes four aspects: the selection of blood vessels and indwelling needle, the fixation and puncture of venous indwelling needle, the sealing of catheter and the control of indwelling time. The implementation of these nursing measures makes the application of intravenous indwelling needle more scientific and reasonable, greatly improves the success rate of puncture and greatly reduces the pain suffered by children during infusion. To sum up, the application of intravenous indwelling needle in pediatric nursing can significantly improve the success rate of puncture and the nursing satisfaction of children, which is worth popularizing.
