The first is to evaluate and measure the nursing level of the poor.

(1) evaluation criteria. According to the comprehensive evaluation of six indicators: independent eating, independent dressing, independent getting out of bed, independent going to the toilet, independent indoor walking and independent bathing. If all six indicators meet the standards, it can be regarded as having the ability of self-care and the nursing level is completely self-care; There are 3 items (including 3 items) below the standard, which can be regarded as partial loss of self-care ability, and the nursing level is semi-nursing; There are more than four (including four) indicators can not meet the standard, can be regarded as a complete loss of self-care ability, nursing level for complete care.

(2) assessment time. From May 5 to May 30, 3/KLOC-0, each township (street) completed the assessment and identification of the self-care ability (nursing level) of the poor people in its jurisdiction, and the District Civil Affairs Bureau will conduct spot checks on the assessment and identification results of the self-care ability in due course.

(3) Evaluation method. Decentralized support for the poor population consists of township (street) village (community) cadres, civil affairs cadres, hospital doctors, village (community) cadres and village (neighborhood) group leaders. , follow the principle of intuition, simplicity and easy operation, combine with the living conditions, carry out face-to-face evaluation and appraisal of the self-care ability of the poor in villages and households, and truthfully fill out the Self-care Ability of the Poor in Dachuan District of Dazhou City. The centralized support for the poor is composed of township (street) leaders, civil affairs cadres, hospital doctors, vice presidents of nursing homes and other staff, and the evaluation method is consistent with the decentralized support for the poor. Two, the standards and methods of nursing expenses for poor people.

(1) nursing standard. Poor caregivers who have the ability to take care of themselves will not pay the nursing fee for the time being. The standard of nursing (half nursing) for some poor caregivers who have lost the ability to take care of themselves is 200 yuan/person-month, and the standard of nursing (full nursing) for all poor caregivers who have lost the ability to take care of themselves is 400 yuan/person-month.

(2) Nursing and nursing methods. For the poor people with scattered support, nurses provide daily care, life care and hospitalization care services in the form of government-purchased services. Centralized support for poor people, by the support service agencies responsible for providing daily care, life care and hospitalization care services.

(3) caring for nursing staff. Township (street) people's governments (offices) and village (neighborhood) committees should urge and help some poor people who have lost their ability to take care of themselves and completely lose their ability to take care of themselves to implement the nurses one by one, and sign the Nursing Agreement on Decentralized Support for Poor People in Dachuan District of Dazhou City (Annex 2) with the nurses to stipulate the nursing standards, nursing contents and other related matters. Nursing staff must have relatives or other persons with full capacity for civil conduct, under the age of 65, in good health and capable of nursing. Village (community) cadres can't be nurses in principle. Care and nursing staff for other reasons lose the ability of care and nursing or fail to perform the obligation of care and nursing, the township (street) people's government (office) and the village (neighborhood) committee shall promptly urge and help poor people to re-implement the care and nursing staff. Poor mental patients are sent to mental hospitals for treatment by government departments, and nursing fees are not paid to nursing staff.

(four) the way to pay the nursing fee. Decentralized support for poor people's care and nursing expenses will be distributed to nursing staff on a quarterly basis; The nursing fee for centralized support of destitute personnel shall be punched directly to the support service institutions on a quarterly basis, which shall be uniformly used for the nursing fee, and the use of funds shall not be changed. At the end of each quarter, each township (street) will submit the Summary of Care Fee Payment for Poor People in XX Township (street) in Dachuan District (Annex 3) and the Summary of Care Fee Payment for Poor People in XX Nursing Home in Dachuan District (Annex 4) to the District Civil Affairs Bureau for monthly subscription.

Third, the job requirements

(1) Strictly implement dynamic management. The newly-added destitute personnel should be evaluated and appraised for their self-care ability (nursing level) at the same time; When the self-care ability of the poor people changes due to illness, disability or other reasons, the village (neighborhood) committee shall report to the township (street) people's government (office) in a timely manner, re-evaluate their self-care ability (nursing level), and adjust the nursing fee standard of the poor people in a timely manner. If the destitute personnel terminate the assistance and support due to death or other reasons, they will stop paying the assistance and support and nursing fees from the next month after the termination of the assistance and support.

(two) to strengthen the supervision and management of the responsibility of caring for the poor. At the end of each quarter, the township (street) people's government (office) should organize personnel to pay a return visit to the poor people in this area and evaluate the performance of their nursing agreement. Village (neighborhood) committees should establish a contact mechanism for special personnel, often visit the poor people who are scattered and supported, track and understand the performance of the nursing agreement, understand the basic living conditions and physical conditions of the poor people in their respective areas, and master the service of the nursing staff (if the nursing staff do not perform their duties correctly, they should be replaced in time). For fraud, fraud, interception, misappropriation, misappropriation, dividing the nursing expenses of poor people, resulting in serious consequences due to dereliction of duty, graft, buck passing, etc. , to seriously investigate the responsibility of the relevant personnel, to find out the illegal behavior, will be reported to the relevant departments in accordance with the relevant provisions, according to the law and discipline dealt with severely.
