The first is to evaluate and measure the nursing level of the poor.

(1) evaluation criteria. According to the comprehensive evaluation of six indicators: independent eating, independent dressing, independent getting out of bed, independent going to the toilet, independent indoor walking and independent bathing. If all six indicators meet the standards, it can be regarded as having the ability of self-care and the nursing level is completely self-care; There are 3 items (including 3 items) below the standard, which can be regarded as partial loss of self-care ability, and the nursing level is semi-nursing; There are more than four (including four) indicators can not meet the standard, can be regarded as a complete loss of self-care ability, nursing level for complete care.

(2) assessment time. (3) Evaluation method. (2) Nursing and nursing methods. For the poor people with scattered support, nurses provide daily care, life care and hospitalization care services in the form of government-purchased services. Centralized support for poor people, by the support service agencies responsible for providing daily care, life care and hospitalization care services.

(3) caring for nursing staff. Township (street) people's governments (offices) and village (neighborhood) committees should urge and help some poor people who have lost their ability to take care of themselves and completely lose their ability to take care of themselves to implement the nurses one by one, and sign the Nursing Agreement on Decentralized Support for Poor People in Dachuan District of Dazhou City (Annex 2) with the nurses to stipulate the nursing standards, nursing contents and other related matters. Nursing staff must have relatives or other persons with full capacity for civil conduct, under the age of 65, in good health and capable of nursing. Village (community) cadres can't be nurses in principle. Poor mental patients are sent to mental hospitals for treatment by government departments, and nursing fees are not paid to nursing staff.

(four) the way to pay the nursing fee. Decentralized support for poor people's care and nursing expenses will be distributed to nursing staff on a quarterly basis; The nursing fee for centralized support of destitute personnel shall be punched directly to the support service institutions on a quarterly basis, which shall be uniformly used for the nursing fee, and the use of funds shall not be changed. If the destitute personnel terminate the assistance and support due to death or other reasons, they will stop paying the assistance and support and nursing fees from the next month after the termination of the assistance and support.

At the end of each quarter, the township (street) people's government (office) should organize personnel to pay a return visit to the poor people in this area and evaluate the performance of their nursing agreement. Village (neighborhood) committees should establish a contact mechanism for special personnel, often visit the poor people who are scattered and supported, track and understand the performance of the nursing agreement, understand the basic living conditions and physical conditions of the poor people in their respective areas, and master the service of the nursing staff (if the nursing staff do not perform their duties correctly, they should be replaced in time).
