1, certification application and contract;

2. Business license and organization code certificate (only business license can be submitted when the five certificates are combined);

3. Qualification or license;

4. Internal audit and management review;

5. Environmental monitoring report within one year;

6. EIA report and acceptance reply (whether it is necessary to implement "three simultaneities"); (Enterprises established after the EIA Law in September 2003 need to do EIA, and previous enterprises need to do EIA if they are seriously polluted, and construction, property and sales need monitoring and EIA);

7. List of environmental factors, laws and regulations and other requirements;

8. Law-abiding certificate (EIA information is complete and not provided);

Conditions to be met

1) The enterprise has obtained the legal person qualification (or part thereof) registered by the State Administration for Industry and Commerce or relevant institutions, and conducts annual inspection within the specified time;

2) Having obtained the administrative license documents stipulated by relevant laws and regulations (if applicable), and being able to provide the license or qualification certificate and annual inspection certificate within the validity period;

3) The products or services produced and processed meet the requirements of relevant laws and regulations, safety and health standards and relevant specifications of People's Republic of China (PRC);

4) According to the above basic certification basis and relevant special technical requirements, a documented environmental management system was established and implemented. Generally, the system needs to run effectively for more than 3 months;

5) Environmental safety monitoring report (environmental management system only);

6) Reports and replies of "environmental assessment" and "safety assessment", and "three simultaneities" acceptance report (only environmental management system if necessary);

7) Geographical location and plane schematic diagram of the factory;

8) Schematic diagram of main process flow and pollutant output;

9) Certificate of pollutant discharge implementation standards (if necessary, only environmental management system certificate);

10) The applicant has no major accident certificate within one year (if necessary);

1 1) Note: Construction, municipal and supervision companies require qualification certificates and safety production licenses; Food production enterprises require QS certificate; (except all products are exported); Household appliance manufacturers require CCC certificates (except for all products exported); As well as enterprises that the state explicitly requires permission for the types of enterprises that need to apply, relevant certification materials must be submitted.
