How to write for the mentally retarded in the residential property preparation report

Fill in the following contents for residential property of mentally retarded persons:

1. The guardian shall submit the following applications: name, gender, date of birth, schooling, work, marital status, housework, onset process, onset age, treatment process, diagnosis conclusion, as well as the guardian's name, ID number and proof of relationship with the disabled.

2. The community, village, sub-district office and township shall issue the following certification materials: the performance of the patient in the community and village after the onset, whether there are any situations that require the signature and seal of the community, village, sub-district office and township, such as injury, destruction of things, swearing and relations with neighbors.

3. Relevant medical proof materials: patients hospitalized in mental hospitals carry copies of hospitalization medical records, patients who are not hospitalized carry outpatient medical records of mental hospitals at or above the municipal level and the diagnosis certificate of the hospital, and patients who have been taking medicine in mental hospitals for two consecutive years can carry the proof materials of the hospital.

4. Original and photocopy of my ID card and household registration book.
