How to be a specialist nurse
In recent years, specialist nurses have attracted attention, especially stoma nurses. Being in a grassroots hospital, I also want to be a specialist nurse. I have been working in the Department of Orthopedics and Traumatology for 16 years, and I would like to share some thoughts here for reference by colleagues.
My association with the Department of Orthopedics and Traumatology should be counted from the time I received a patient with a severe right thigh fracture at the Department of Surgery at Beachhead Hospital in 1994, because that was the male patient who I had the deepest impression of, and through my participation in After rescue, treatment and meticulous care, I wrote two articles: "Love Gathers Here", which was published in the comprehensive edition of "Contemporary Nurse", and "First Aid and Nursing Care of a Patient with Severe Right Thigh Fracture". He first participated in the "National Emergency Department" "Academic Conference on Emergency and Critical Care Nursing" was published in the academic edition of "Contemporary Nurse". From then on, I had a preliminary understanding of the care of orthopedic patients. To truly have a comprehensive understanding, practice, thinking and summary of patients in the Department of Orthopedics and Traumatology, we should remember that we officially started working in the Department of Orthopedics and Traumatology in 1997. I was transferred to the Longhui County Hospital of Traditional Chinese Medicine in 1995 and rotated in the Department of Internal Medicine, Department of Orthopedics and Traumatology, and the Nursing Department for three years, and finally stayed in the Department of Orthopedics and Traumatology No. 1. The first Department of Orthopedics and Traumatology was mixed with the Department of Obstetrics and Gynecology and the Department of Burns, and had to accept different types of patients every day. Patients in the orthopedics department and burns department are arranged in beds 1-40, while the obstetrics and gynecology department only has 10 beds, namely beds 41-50. When traffic accidents occur frequently, extra beds have to be added or beds in the obstetrics and gynecology department are occupied. When there is a gas explosion in a coal mine or a fire in a firecracker factory, there are many burn patients. In addition to being assigned to the undergraduate department, they will also be assigned to other departments. During the family planning campaign, there were many patients in the obstetrics and gynecology department... Orthopedics and traumatologists (also burn surgeons) and obstetricians and gynecologists often had conflicts. There were few nurses, so they were caught in the middle, which was particularly uncomfortable.
With the continuous expansion of the scale of the hospital, the Orthopedics and Traumatology Department of our hospital has transformed from the original chaotic situation into the key specialty of Hunan Province today: it has four independent orthopedics and trauma departments, namely Orthopedics and Traumatology. The first department (mainly treats upper limb bone injuries and osteomyelitis), the second orthopedic department (lower limb bone injuries), the third orthopedic department (craniocerebral and spinal bone injuries), and the fourth orthopedic department (minimally invasive department for lumbar disc herniation). Therefore, specialist nursing in orthopedics and traumatology is particularly important, and cultivating specialist nurses is the general trend.
To become a qualified specialist nurse, you should do the following: 1. Take the initiative to learn specialist nursing knowledge and techniques. 2. Strengthen the training of nurses in the hospital and improve the overall level. 3. Strive for leadership’s attention and carry out special care. 4. Practice diligently, think more, and be good at summarizing. 5. Have love, enthusiasm, carefulness, perseverance and sense of responsibility. 6. Correctly understand the importance of specialist nursing and advance despite difficulties. (Article reading website: www.sanwen.net)
Now, based on reality, let’s talk about how I achieved the above six points.
Any knowledge and skills are learned, not given by others. Therefore, if you want to be a good orthopedic nurse, you must take the initiative to learn the treatment and nursing knowledge of orthopedic diseases. During the study process at three levels: health school, advanced nursing class, and self-study undergraduate, I knew very little about orthopedics and traumatology. While working at Tantou Hospital, I spent most of my time in the Department of Internal Medicine (8 years, treating patients of all ages). patients). Orthopedics and traumatology is a specialty, but patients vary in age and have different comorbidities. Most patients with upper limb fractures are younger, while patients with lower limb fractures are older, especially femoral neck fractures. Most of the patients are elderly and have multiple medical diseases. Therefore, I have eight years of clinical experience as a nurse in the internal medicine department. It gives me the confidence to deal with the medication care, life care, psychological care, etc. of orthopedic patients. However, for special treatment of patients, such as small splint fixation, plaster external fixation, bone traction, skin traction, surgical internal fixation, and bracket external fixation. and functional exercises, etc., you must humbly consult an orthopedic surgeon for advice. At the same time, go to the bookstore to buy specialist nursing books such as "Orthopedic Nursing" and compare them with the books, while studying, thinking and recalling various fracture patients encountered in clinical practice. Keep at it, and slowly, you will be able to judge whether the patient has a fracture, and how to deal with a patient with a fracture.
The development of any new technology is inseparable from the unified planning and implementation of the Nursing Department.
Being a specialist nurse does not mean that you will automatically become a nurse after working as a nurse in any specialty for several years. You must know the relevant knowledge of diseases in that specialty, including their etiology, clinical manifestations, examination, diagnosis, treatment, nursing, rehabilitation and a series of theoretical knowledge. It is also necessary to master relevant techniques such as hemostasis, bandaging, fixation, puncture, external application of traditional Chinese medicine, fumigation, massage, etc., that is, master basic nursing techniques (bed making, intramuscular injection, venipuncture infusion, blood transfusion, catheterization, enema, oxygen delivery, sputum suction) , oral care, skin care, bowel care, etc.), it is also necessary to comply with the needs of orthopedic and traumatology specialty care, meet the health needs of patients, and provide patients with physical, psychological, spiritual, cultural and other considerations. Holistic care (routine internal medicine plus specialist care). Only through systematic specialist training for all nurses in the hospital can we be targeted and adapt to the needs of temporary transfers to departments and frequent transfers of nurses (temporary nurses often participate in the recruitment and deployment of hospital nurses).
The hospital is a whole. To realize the importance of nursing work, we must win the support of hospital leaders. The Department of Orthopedics and Traumatology, like other departments, used infusion as the main treatment method. From the time the patient was admitted to the hospital, several bottles of medicine were infused every day. This not only caused physical and economic losses to the patient (the cost of antibiotics was high, and it was harmful to the liver and kidneys). , damage to the blood system), and brings risks and loads to nurses (drug allergies and other adverse reactions, it takes time for nurses to take medicines, dispense medicines, change suspension bottles, remove needles, observe changes in the condition and record them). When TCM treatment and care are carried out, patients generally respond well. For example, when the traditional Chinese medicine Sanyu Xiaotong Ointment is used externally for one to three days, the swelling and analgesic effect is obvious. It is much stronger than the swelling effect of mannitol injection and glycerol fructose injection. It has no side effects and the cost is halved. Local treatment in the department of orthopedics should be the main one. After reduction and fixation, it is necessary to reduce swelling quickly, reduce pain, increase nutrition, and carry out step-by-step functional exercises to promote fracture healing and recover as soon as possible.
No matter which position you work in, if you want to achieve results, you must work hard with your hands and feet, think frequently, be good at discovering deficiencies, and summarize successful experiences at work. We face patients with fractures every day. From admission to discharge, where did the patient fracture, to what extent, what examinations were performed, what drugs were used, and what treatments were adopted? How are we doing in terms of care? How should I respond if I encounter the same patient or a patient with a more severe condition in the future? Ask yourself more in your mind, what have I gained from working in the Department of Orthopedics and Traumatology for such a long time? Why do some patients recover slowly from fractures? Why are elderly patients prone to complications such as pressure sores, pneumonia, urinary tract infections, joint stiffness, and venous thrombosis? As long as we work hard, we will have new insights every day. After working in the Department of Orthopedics and Traumatology for a long time, I had the idea to write about it, such as "Application of Nursing Procedures in Elderly Patients with Femoral Neck Fracture", "A Case of Replantation of a Broken Limb and Dissection of Both Knees" "Psychological Nursing", "Perioperative Nursing of Patients with Multiple Fractures of the Limbs and Shock", etc. were successively written and published in "Contemporary Nurses".
Being a specialist nurse requires love, enthusiasm, care, perseverance and responsibility. People without love will lose their temper or blame others for patients and their families. Without enthusiasm, they will not be able to write nursing papers. Without care, they will not be able to observe the special changes of patients. If you have a sense of responsibility, you will not tire of mediating various doctor-patient conflicts and prevent the occurrence of medical disputes. It can be said that I love the nursing profession, whether it is internal medicine or orthopedics. I love every patient from beginning to end, no matter where they come from, rich or poor, senior officials or illiterate, old or children, as long as their needs are reasonable, I will Try to satisfy them. I have been working in the Department of Orthopedics and Traumatology for 16 years. Whether it is a backward old inpatient department or a modern new inpatient building, I always keep my head down and do things. During work, I strive to spend time on patients, and after completing my job, I help patients solve their problems. Practical problems, such as buying meals, turning on water, changing sheets, explaining to eliminate doctor-patient conflicts, guiding patients to eat reasonably, keep a good mood, take medicine correctly, and insist on functional exercises, etc.
Being a specialist nurse is sometimes not your own wish, but the need of the unit. After the Orthopedics and Traumatology Department of our hospital became a key specialty in Hunan Province, I feel that I should strengthen my study and cultivation to be worthy of the hospital and patients. The hospital is our foothold. Without the support of the big family of the hospital, it would be difficult for us nurses to use our talents.
Through various stages of study in health schools, advanced nursing classes and self-study exams, our theoretical knowledge of nursing has been improved step by step. However, it is only through the hospital that we can put the knowledge and skills we have learned into play, that is, to test the effectiveness of our learning. , otherwise, it’s just reading in vain. Patients are our service objects and our foundation. All the knowledge and techniques we have learned are to protect life, relieve pain, and improve health. Without patients, there would be no doctor. Nurses work in hospitals and fight side by side with doctors. Whether patients call us doctors or nurses, we have the same name - angels in white. Orthopedic and traumatology patients have temporarily lost their labor force due to injuries, and physical losses, psychological setbacks, and impacts on family and social aspects follow one after another. The importance of orthopedic and traumatology care is to do our best to help patients as soon as possible. Rehabilitation, reintegration into society, normal work and life.
Of course, it is difficult to be a specialist nurse. As long as we build up confidence and courage, and advance despite difficulties, we will move away from the difficult words, from being unsuitable to adapting, and then from adapting to being handy and proficient, thus becoming a true specialist nurse.
