Baoji Medical Insurance Retail Pharmacy Application Form

Retail pharmacies that are willing to undertake designated services for basic medical insurance for urban employees can submit a written application to the municipal labor and social security administrative department and provide the following materials: (1) Pharmaceutical business enterprise license and business license Copies and photocopies; (2) Proof of GSP certification; (3) Pharmacist registration certificate or qualification certificate and the original and photocopy of the labor relationship certificate with the retail pharmacy; (4) Corresponding medical insurance management system and drug operation List of varieties; (5) List of retail pharmacy medical insurance managers, pharmacy employees and various professional and technical personnel; (6) Other materials specified by the municipal labor and social security administrative department.

The format of the application form is the same as that of the ordinary application form.
