How to report the loss of a bus card

Bus card collection:

(1) When each department applies for a bus card, it is required to fill in the Bus Card Collection Application Form (see Annex 3 for details), which shall be signed and approved by the department head;

(2) After receiving the Application Form for Departmental Bus Card and submitting it to the person in charge of the Personnel Administration Department for signature and confirmation, the Administrative Commissioner will issue the bus card to the application department for use;

(3) When the accepting department returns the bus card with insufficient balance to the personnel administration department to reissue the card, it shall sign the return time on the registration form for accepting the bus card by the administrative commissioner, and submit the Registration Form for Using the Bus Card to the administrative commissioner for review;

(4) The Administrative Commissioner is responsible for verifying whether the accounts are consistent with the facts and checking whether the bus card is damaged. If it is damaged or demagnetized by the user department, the user department shall bear the corresponding expenses. If the verification passes, the bus card will be fully recharged according to the application process.
