Go to Fuzhou Civil Affairs Bureau (Disabled Persons' Federation) for binding.

After formal inquiry, bring three bareheaded photos with white background, ID card, household registration book and hospital certificate to Fuzhou Civil Affairs Bureau (Disabled Persons' Federation) for binding.

The holder of the disabled card shall keep it properly, and it shall only be used by himself, and shall not be altered or lent.
