Can a foreigner apply for a disability certificate in Kunshan?

The disability certificate needs to be handled at the place where the household registration is located.

Processing flow:

1. The applicant holds his/her ID card, household registration book and three recent 2-inch bareheaded color photos to apply to the Disabled Persons' Federation in the neighborhood where his/her household registration is located. (To apply for visual disability, you must show the recent binocular vision diagnosis certificate issued by the hospital)

2. If the Jie Zhen Disabled Persons' Federation fails to meet the disability assessment criteria in the first instance, it shall inform the reason for termination on the spot. Those who meet the disability assessment standards shall fill in the Application Form for Disabled Persons in People's Republic of China (PRC), the Disability Assessment Form in People's Republic of China (PRC) and the Statistical Questionnaire on Basic Information of Disabled Persons in XX.

3. The applicant holds the "People's Republic of China (PRC) Disability Assessment Form" to the District and County Disabled Persons' Federation for examination and seal. The staff of the district and county disabled persons' federations help them make an appointment to go to the hospital for disability assessment according to the disability situation. The applicant shall go to the designated hospital for disability assessment on the specified date with the People's Republic of China (PRC) Disability Assessment Form.

4, the designated hospital is responsible for vision, hearing, speech, physical disability, mental and intellectual disability assessment. According to the evaluation results, the district and county disabled persons' federations sign opinions on disabled persons who meet the disability standards, and print disability certificates after passing the examination.

5. After the disability certificate is examined and sealed by the Municipal Disabled Persons' Federation, the district and county disabled persons' federations will notify the street disabled persons' Federation to receive it. Jie Zhen Disabled Persons' Federation informed the applicant to come and collect the disability certificate.
