202 1 Announcement on Receiving the First Batch of Teacher Qualification Certificates in Yangxin County, Binzhou City, Shandong Province

Qualified teachers in kindergartens, primary schools and junior high schools who were recognized online and on-site from April 2002 1 April 2003 13 to April 2005 15 (see Annex1for the on-site self-collection list and Annex 2 for the mailing list).

Second, the time and place of collection.

1, collection time: April 26th to 27th (8: 30am-65438+ 02: 00pm, 65438+04: 00pm-65438+07: 30pm).

2. Pick-up location: Windows 5 and 6 of Yangxin County Administrative Examination and Approval Service Bureau (North Gate of Government Affairs Service Center, No.567, Yangcheng Sixth Road) (you can take bus No.2 and No.5 and get off at Yangxin County Government Affairs Service Center Station).

Third, the way of receiving

Distribution shall be made according to the method selected by the applicant during online and on-site identification.

1. I collect it: I take my original ID card to the county government service center to collect it.

2. Entrust others to collect it: take the copy of the client's ID card, the original of the client's valid ID card and the power of attorney to the county government service center to collect it (see Annex 3 for the power of attorney template).

3.EMS mailing: Applicants who voluntarily choose the mailing method during online and on-site confirmation will provide EMS mailing free of charge. The mailing address and contact information shall be subject to the reserved information confirmed online and on site.

Fourth, matters needing attention

1. The materials received this time include 1 teacher qualification certificate and 1 teacher qualification application form. Please take good care of it by the applicant or the trustee. If it is lost, you will be responsible for it. The teacher qualification application form has the same legal effect as the teacher qualification certificate and needs to be stored in personal files. Please submit it to the applicant's file management department in time and store it in my personal file.
