Receiving and filling out forms

Collect application forms and cost of production.

Please ask the manager of the insured unit to collect the completed Application Form of Guangzhou Social Security (Citizen) Card from the new insured person before the next month of insurance registration 19, and collect the social security (citizen) card fee of 20 yuan/person at the same time.

The handler shall review the application form to ensure that the information filled in is true and complete, and finally stamp the corresponding position of the application form for confirmation. The number of recipients should be consistent with the number of new insured persons last month.

Hand in the form, pay the fee and get the card.

The medical insurance bank will notify the insured units to go through the formalities of submitting forms, paying fees and getting a card at Guangzhou Medical Insurance Center after the registration month 19.

1. People's Republic of China (PRC) General Tax Notice issued by the local taxation bureau in the current month and its copy or electronic receipt and its copy of bank receipt;

(2) Procedures for submitting forms, paying fees and obtaining cards:

2. Pay the fee. Pay the business card printing fee at the social security (citizen) card payment window with the Notice of Payment of Charge Fund. After the payment is successful, ICBC will issue a unified bill for administrative fees in Guangdong Province.

Insured units can pay in cash or by cheque. If the production expenses are paid by cheque, the name of the payee should be filled in as "Guangzhou Financial Collection", and the unit manager must go to Guangzhou Medical Insurance Center to receive the unified bill of administrative fees in Guangdong Province within three working days after completing the payment procedures by cheque, and get the card.

(3) Precautions

If, due to special reasons, the actual number of cardholders in the insured unit is inconsistent with the number registered last month, please ask the unit manager to go to the social security (citizen) card business window of Guangzhou Medical Insurance Center to confirm the number of cardholders within 5 working days before the registration next month 19. When handling, please ask the agent to bring the following materials: personal identity card and its copy, and the list of resigned persons (stamped with the official seal of the unit).
