Conditions for outpatient referral:

1. Participation in Dongguan medical insurance in accordance with the regulations;

2. Visiting the designated outpatient medical center and needing to be referred for treatment due to medical condition.

Outpatient referral information:

3, the patient's own ID card (if the new social security card has not been issued with ID card).

Outpatient referral process:

1. First of all, the insured person goes to the established outpatient medical point for medical treatment;

2. The attending doctor conducts diagnosis and treatment, and according to the needs of the condition, if the patient needs to be referred to the clinic for medical treatment, he or she will apply for referral and fill in the referral slip;

3. "Referral form", social security card and ID card to the township community health center for medical treatment.

Remarks: If you need to be transferred to a higher-level medical institution for outpatient treatment due to your condition, you can have the "referral form" issued by the designated medical point, and submit it to the township community health center for approval, and then transfer it to the main outpatient department of the fixed-point medical institution in the city for treatment.

Supplementary note: outpatient treatment at the township community health center after referral can be directly handled on-site settlement and reimbursement. The participant inserts the new social security card into the password verification reader at the fee collection counter, and enters the password of the new social security card for password verification, and the on-site settlement and reimbursement can be completed after the password verification is passed. Note that participants who have not yet been issued with a new social security card do not need to verify their passwords.
