If there are multiple social security numbers for the same social security insurance, you need to apply for the merger of social security numbers according to the following requirements:

I. If the personal information is correct, the merger of personal social security numbers will be directly handled, and the social security department shall be responsible for the merger first, and then the applicant shall go to the local tax department to handle the relevant matters with the confirmation form issued by the social security department and the relevant information. Secondly, if an individual has multiple social security numbers and the personal information corresponding to each number does not match, he or she needs to change his or her social security personal information at the local tax department before going to the social security department for the merger of social security numbers.

After the change of personal information is completed, the social security number will be merged by the social security department, and then the applicant will go to the local tax department with the confirmation form issued by the social security department and relevant information to handle the related matters.

Third, the local tax department for the merger (modification) of personal social security numbers need to collect information:

1. personal identity documents and copies (copies need to be signed and fingerprinted);

2. entrusted to do the work on behalf of the agent, the agent needs to show the party's letter of attorney (signed and fingerprinted), the agent's identity documents and copies (copies need to be signed and fingerprinted);

3. One copy of "Confirmation Form for Merger of Duplicate Participants" (collected for the business of merging individual social security numbers);

4. Two copies of "Application Form for Modification of Individual's Key Information";
