Procedures for enrollment:

Go to the individual contribution window of the social security institution to get and fill in the Individual Contribution Declaration Form of Social Insurance of So-and-so City and the Contract of Entrusting Banks to Collect Social Insurance Premiums;

Hold the Contract of Entrusting Banks to Collect Social Insurance Premiums to the account opening bank to affix the seal;

Hold the information required for enrollment, the Individual Contribution Declaration Form of Social Insurance of So-and-so City and the Contract of Entrusting Banks to Collect Social Insurance Premiums. Individual Contribution Declaration Form" and "Contract Letter of Entrusting Banks to Collect Social Insurance Premiums" to the individual contribution window of the social security institution in the district where the household registration is located;

If you need to apply for the "So-and-so Municipal Labor Security Card", you have to come in person to apply for the card and provide a copy of your ID card (checking the original) as well as the return receipt of the digital photo taken by the second-generation ID card networked photographic studio recognized by the municipal public security organs.

Other notes: participants should be in the bank before the 21st day of each month to deposit the full amount of social security contributions due for that month in case of collection failure. If the collection is unsuccessful for three consecutive months, the social security organization should stop the insurance for them, and if they re-enroll in the insurance, they must declare again.
