Shanghai social security card loss reporting and replacement process

I. Report the loss of social security card

Cardholders can temporarily report the loss by calling the social security card service telephone number (telephone number: 962222), or they can formally report the loss at community acceptance outlets with valid identity documents. The social security function will be frozen immediately after the loss is reported.

Remarks: Temporary loss reporting is valid for 5 days. If the cardholder fails to report the loss officially within the temporary reporting period, the social security function will be automatically restored after the expiration of the validity period; If the cardholder handles the formal loss reporting or directly handles the formal loss reporting within the validity period of the temporary loss reporting, the social security function will stop being used.

Bank loss reporting: If the social security card has been enabled with the financial service function, the cardholder can make a temporary loss reporting by calling the bank service number, or take a valid ID card to the bank acceptance outlet to make a formal loss reporting. The specific rules for the period of validity of loss reporting, cancellation of loss reporting and replacement shall be determined by the relevant service bank and informed to the cardholder. Second, the social security card replacement

Cancellation of loss reporting and replacement: if the original social security card is retrieved after loss reporting, the cardholder can cancel the loss reporting at the community acceptance outlets with valid identity documents, and the social security function will be restored immediately. If the original social security card cannot be recovered after it is officially reported lost, the cardholder can go through the replacement procedures at the community acceptance outlets or bank acceptance outlets with valid identity documents.

Redemption: If the social security card is defaced, incomplete, unrecognizable, or can't be used normally in the application terminal, the cardholder can go through the redemption procedures at the community acceptance outlets or bank acceptance outlets with valid identity documents.

Re-application: In any of the following circumstances, the cardholder shall re-apply in accordance with the relevant provisions of these Measures.

1, renamed;

2. Change the social security number;

3. Change photos;

4. Change the service bank.

Children over the age of seven should get a social security card with photos printed on it.

Remarks: Shanghai Information Service Center shall complete the production and distribution of social security cards within 15 days after receiving the replacement information.

After the social security card is reissued, replaced and reapplied, the original card is invalid and cannot be restored.
