How to fill in the social insurance registration form
1 For the insured unit that has obtained the industrial and commercial business license, the social insurance registration form shall fill in the license type, license number, issuance date and validity period in the column of "industrial and commercial registration license information";
For the insured units that have not obtained the business license but have been approved by the relevant departments, the Social Insurance Registration Form shall fill in the approval unit, approval date and approval number in the column of "Information on Approval of Establishment", that is, according to the approval documents of the relevant departments.
Every enterprise needs to fill in a social security agency to ensure the correct transfer of registration information and payment information between local tax and social security agencies.
