Report the loss of social security card (provincial level)

In case of emergency, you can report the loss of the social security card bank account (magnetic stripe account) orally through the customer service hotline of Industrial Bank at 9556 1, but you should go through the formal written loss reporting formalities at the business department of the head office of Industrial Bank with valid identity documents within 5 days, otherwise the oral loss reporting will automatically become invalid.

Social Security Card Replacement (Provincial)

When the social security card of the provincial insured is lost, stolen or damaged, the cardholder should first change the card with his valid ID card and social security card (in case of damage) to Fujian Medical Insurance Management Center. Fujian Medical Insurance Management Center will make a new social security card for the cardholder after reporting the loss, and then the cardholder will go to the business department of Industrial Bank with the newly reissued social security card and his valid ID card to initialize the financial e-wallet and activate the card. After changing the card, Industrial Bank will open a new bank account for social security card customers.

Fujian Medical Insurance Management Center has a one-time charge for card replacement, and the charge standard is 25 yuan/card.

Due to the change of bank card account number, the new social security card applied for by changing the card after reporting the loss needs to re-apply for the business functions such as financial management and withholding in the old social security card.
