Guangzhou's old medical insurance card is replaced with a new one.

The old medical insurance card must be replaced by a new one in the bank, and the second-generation social security card must be registered and replaced in the bank.

Materials required for medical insurance card:

1. Two recent two-inch bareheaded photos of the insured. If the insured is a preschool child, provide two recent two-inch bareheaded photos of mother and son or father and son;

2. The original and two copies of the insured's ID card;

3. The original and two copies of the household registration book of the insured unit, and one copy of each page of the household registration book;

4. The original and photocopy of the valid certificate and other relevant materials of other family members except the insured person in the household registration book; For example, if family members have provincial and municipal medical insurance books, you only need to bring the original medical insurance book and copy the first page.
