1, original and photocopy of marriage certificate;
2. Original and photocopy of the Birth Medical Certificate;
3. One original and one copy of the baby's birth certificate;
4. Original and photocopy of medical diagnosis certificate;
5 copies of the front and back of the second-generation ID cards of both men and women;
6. Non-local employees shall provide the original and photocopy of Beijing Work Residence Permit, and the Contact Sheet of Maternity Service for Non-local Personnel in Beijing;
7. Beijing Birth Registration Form in duplicate;
Note: The above copies must be made on A4 paper and stamped with the official seal of the company.
(2) Processing time: May 25th of each month (the month after the baby is born).
(III) Handling process: The insured unit shall submit the above materials to the payment department on the second floor of the social security handling hall from the 5th to 25th of each month. When applying for maternity allowance for the last time in the current month, you should also submit the Monthly Statement of Maternity Allowance Payment for Insured Workers in Beijing (Health Statement II) (in duplicate). In line with the conditions of enjoying the late childbirth bonus allowance, both husband and wife choose the object of enjoying the late childbirth allowance in the birth registration form, and both husband and wife sign and confirm, and affix the seals of both parties.
(1) If both husband and wife have participated in maternity insurance, the female unit shall fill in the maternity registration form.
(2) If the woman has not participated in maternity insurance, and the man has participated in maternity insurance, and the man enjoys maternity allowance, the unit where the man works shall fill in the maternity registration form (in duplicate), and bring the marriage certificate of the male employee and its copy and related materials to the four insurance payment post for personnel registration.
Note: The person in charge of the unit, the person filling in, the contact telephone number and the date of filling in the form must be filled in completely. If there is any alteration, please affix the official seal of the unit at the altered place.
