After the digital certificate is lost, the insured unit needs to fill out the paper application form of Beijing Digital Certificate Application Form in duplicate, and go to the digital certificate service network to handle the business of reporting the loss and resending the digital certificate.

Lost and reissued materials:

1. Application Form for Digital Certificate of Beijing Company in duplicate, stamped with the official seal of the company.

2. Original (photocopy) of organization code certificate, social insurance registration certificate and business license.

3. Certificate media fee 80 yuan
