Social security card application process

I. Checking personal information and filling out forms

1. First, check whether the information printed on my household registration book, ID card and Shanghai social security card application registration form is consistent.

In case of inconsistency: after filling in the application form according to the instructions, take the household registration book, ID card, application form and relevant certificates to the police station (police station) where the household registration is located, issue the computer-printed household registration certificate of Shanghai Public Security Bureau, and then continue to apply for the social security card.

2. Check whether the household registration information on the application form is consistent with the information recorded by the labor security department and the provident fund management department.

If it is inconsistent with the labor security information, fill in the application form according to the instructions, and pay special attention to the information supplement area, in which the medical insurance object should supplement the resident ID card information and medical insurance card (magnetic stripe) information. The holder of the labor manual shall fill in the information of the resident ID card and the information recorded in the labor manual.

If it is inconsistent with the information recorded by the provident fund management department, the unit shall go through the formalities for correcting the information of the provident fund.

Second, go through the payment procedures.

According to relevant regulations, pay 25 yuan/piece. To apply for outlets, you must issue corresponding expense receipts.

Third, take photos and collect fingerprints.

Take photos with the social security card and collect fingerprints with the application form.

Fourth, get a social security card.

According to the Measures for the Administration of Social Security Cards in Shanghai, if the personal information is correct, it will be distributed to the original application outlets within the prescribed time limit. If there are errors in personal information, the application and distribution period may exceed the prescribed period.
