Where can I reissue the social security card in Sichuan Province?

I lost my social security card. It can be reissued in the first two items of 1.2.

1. business card printing: the original ID card of myself and my agent (the company only needs to provide the official seal of Chengdu Social Security Card Production Application Form for business card printing);

2. Card collection: the original ID card of myself and my agent (the original and photocopy of the cardholder's ID card, and the accepted Application Form for Making Chengdu Social Security Card) need to be collected by the company within seven working days after the card is made;

3. Loss reporting and cancellation: the original ID card of myself and the agent, and the original social security card is required for cancellation of loss reporting;

4. Magnetization and password reset: the original ID card of myself and my agent, and the original social security card of myself.
