Process

1, the insured individual with personal ID card to the bank service window to obtain "Linyi City social security card application registration form".

2. The insured individual will fill in the "Linyi City Social Security Card Application Registration Form" according to the guidance of the bank's front desk staff and paste a copy of the front of the ID card or a photo receipt as required.

3. After handing over the form, the insured individual has to pay the labor cost of social security card (RMB 20/person/card). The bank receptionist will issue a bill to the insured individual.

4. Within 20 working days, each bank notifies the insured individual to collect the card at the application branch.

5. The social security card needs to be activated in order to be used, so please activate it immediately when receiving the card.
