When applying for loss reporting by telephone, the cardholder's name, citizenship number, contact number and other relevant information should be provided.

Twenty-second social security card agencies shall notify the relevant business departments within 1 hour after accepting the application for loss reporting. Relevant business departments should freeze the use of the card within 4 hours after receiving the notice.

Article 24 If a cardholder loses his social security card and applies for a replacement, the original social security card will automatically become invalid from the time when the service outlets of the social security card handling agencies accept the application for replacement.
