Employees who have applied for social security cards, but need to apply for them again after losing them, do not need to submit the receipt of digital photos;

2. Enterprises that have applied for online declaration can directly apply for online certification. First, print the voucher, arrange the receipt of the digital photos of the employees who need to make vouchers according to the order of printing the voucher and attach them to the list, and provide a copy of the employee's ID card, submit it to the business card printing window designated by social security, pay the fee and receive the business card printing receipt;

3. If the online declaration is not made, the enterprise shall submit a copy of the employee's ID card and a receipt of the digital photo of the social security card. If the card is lost, you don't need this receipt. The digital photo receipts of social security cards are arranged in the order of insurance reports and attached to the insurance reports. If the lost social security card needs to be reissued, you can directly fill in the "Application Form for Reissuing the Social Security Card of X City". After the collection department to which the payment belongs prints the voucher list, it can pay at the business card printing window designated by social security and receive the business card printing receipt;

4. If the payment is made in the individual payment window, the original ID card and the receipt of the digital photo of the social security card shall be submitted. If you have already applied for a social security card, you don't need to fill in the form, pay the fee and receive the official card receipt in the personal payment window of the social security institution;
