Registration formalities for the company: 1. Write an application in the name of the unit; 2. Business license, certificate of approval for establishment, certificate of legal person of public institution or other approved practice certificate and its copy; 3, the unified code certificate issued by the national quality and technical supervision department to the organization and its copy; 4. Identity cards and photocopies of the legal representative and responsible person of the enterprise; 5. The name, bank and account number of the bank handling the settlement business of endowment insurance; 6, the unit last year "labor wage statistics annual report" and the unit employees last month salary payment table; 7. Original and photocopy of the insurance contract (or agreement) or the earliest payroll; 8, the insured household registration book, ID card copy, recent one-inch color photos.
