Fill in the forms: social insurance registration form and social insurance payment registration form.

Submissions:

(1) Original and photocopy of industrial and commercial business license or approval from higher authorities;

(2) The original and photocopy of the unified code certificate of the organization (or the legal person code certificate (only referring to individual industrial and commercial households));

(3) Bank and account number of the company (basic deposit account).

Precautions:

(a) the units (including individual industrial and commercial households) insured according to the enterprise method shall also immediately submit the Social Insurance Payment Registration Form to the local tax authorities for relevant procedures.

(2) Individual industrial and commercial households should open a basic deposit account with CITIC Bank in the lobby of the fourth branch of the Local Taxation Bureau before going through the formalities.

(3) The social insurance premium of the new account opening unit in the current month and the monthly payment after 13 shall be directly collected by the social security agency.
